About this file

This document is a State of Arizona Department of Economic Security, Division of Employment and Rehabilitation Services solicitation for a Vending Machine Purchase, Installation, and Training contract. The document (RSA-1488A FORFF) appears to be a pricing and service proposal form for vending machine solutions, with EE Vending Solutions (www.eevendingsolutions.com) as the primary vendor proposing services.

The contract covers multiple vending machine categories including cold beverage, hot beverage, snack, combination snack/beverage, cold food, hot food, and frozen food vending machines. Each category requires specific technical specifications such as ADA compliance, MDB/DEX compatibility, credit card reader options, and guaranteed delivery systems. The vendor is offering a consistent 5% discount off list price across most machine categories. Installation and training services are priced at a uniform $150 per hour statewide, with rates listed for individual Arizona counties. The pricing sheet allows for detailed customization, requiring vendors to specify website catalogs, percentage discounts, and service capabilities for each machine type. Notable machine models referenced include the USI Evoke 10 Cold Drink Machine ($5,369) and Evoke Hot Beverage Merchandiser (Fresh Brew $9,337, Freeze Dried $8,057).
