NOTICE TO BIDDERS

A-iii

This Solicitation Contains a Price Evaluation Factor - The Government will apply a factor of 10% to an apparent low bid submitted by a large business when we also receive a bid from a certified HUBZone small business concern. The 10% factor does not apply to apparent low bids submitted by small businesses. See FAR Clause 52.219-4 Notice of price evaluation preference for HUBZone small business concerns.

In the event this full and open competition results in a contract award to a qualified HUBZone SBC after a price evaluation preference, FAR Clause 52.236-1, Performance of Work by the Contractor does not apply.

§126.700 of the Code of Federal Regulations (Title 13, Part 126, Subpart G), stated below, will replace the performance of work requirements stated in the above mentioned FAR clause.

A qualified HUBZone SBC receiving a contract under this solicitation for general construction must spend at least 50% of the cost of the contract incurred for personnel on its own employees or employees of other qualified HUBZone SBCs. This requirement may be met by expending at least 50% of the cost of the contract incurred for personnel on its employees or it may subcontract at least 35% of the cost of the contract performance incurred for personnel to one or more qualified HUBZone SBCs. A qualified HUBZone SBC prime contractor may not, however, subcontract more than 50% of the cost of the contract incurred for personnel to non-qualified HUBZone SBCs.

Bid Guarantee – Follow the requirements of FAR Clause 52.228-1 Bid Guarantee and Subpart

102.03 of the FP-03. Bid bonds must be originals and have original signatures and corporate seals.

Photocopied and faxed copies of bonds will be rejected and cause your bid to be non-responsive.

ATTENTION: Minority, Women-owned, and Disadvantaged Business Enterprises (DBEs). The Department of Transportation (DOT), offers working capital financing and bonding assistance for transportation related contracts. DOT's Bonding Assistance Program (BAP) offers bid, performance and payment bonds on contracts up to $1,000,000. DOT's Short-Term Lending Program (STLP) offers lines of credit to finance accounts receivable. Maximum line of credit is $500,000 with interest at the prime rate. For further information, call (800) 532-1169. Internet address:

http://osdbuweb.dot.gov.

Accuracy and Completeness of Bid- The Contractor is fully responsible to verify that all data is correct when an offer is submitted. Failure to properly update your data may cause the offer to be rejected.

Request for Technical Information – Questions relative to the plans and SCRs for this solicitation will only be accepted in writing (see Block 9 on Page A-3).

TECHNICAL QUESTIONS REGARDING THIS SOLICITATION WILL NOT BE

ACCEPTED AFTER 4 P.M. ON DECEMBER 8, 2015

WELDERS - Receive rate prescribed for craft performing operation to which welding is incidental.

Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).

Union Rate Identifiers

A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example:

PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.

Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.

Survey Rate Identifiers

Classifications listed under the "SU" identifier indicate that

D-6 no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.

Survey wage rates are not updated and remain in effect until a new survey is conducted.

Union Average Rate Identifiers

Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.

A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.

FLAGSTAFF TO VISTA OVERLOOK

COCONINO NATIONAL FOREST

COCONINO COUNTY, ARIZONA

INTRODUCTION

Project Description

This report presents the findings of a visual, limited subsurface investigation and provides preliminary geotechnical recommendations for the repair, reconstruction, and widening of AZ FLAP 89(1) Flagstaff to Vista Overlook, known locally as State Route 89 or Oak Creek Canyon Road. A Project Vicinity Map and Project Location Map are presented in Figure 1 of Appendix A.

The Central Federal Lands Highway Division (CFLHD) of the Federal Highway Administration (FHWA), in cooperation with the Arizona Department of Transportation and National Forest Service, is proposing to improve traffic operations on an 8.4 mile segment of the route by widening the roadway to provide adequate shoulders, flattening side slopes, extending drainage structures, and constructing passing lanes. This project is located immediately south of Flagstaff, Arizona, within the Coconino National Forest. The route extends from Flagstaff in a southerly direction through Oak Creek Canyon and Sedona, Arizona.

This project will consist of 4R improvements over the 8.4 miles programmed for reconstruction. The proposed improvements include widening the roadway template to 40 feet with two 12-foot travel lanes and 8-foot-wide paved shoulders. There are three major structures located within the project limits. Two of the structures are reinforced concrete box culverts, while the other structure is a reinforced concrete arch culvert. All three structures will be extended to accommodate the proposed roadway widening. Additional safety improvements are proposed along the route and include construction of various passing and turn lanes. Cross drainage structures will also be improved to accommodate the proposed widening, as well as ensure they meet hydraulic design requirements.

The main focus of this preliminary geotechnical report is to provide information for the development of the 30% preliminary plan set and provide recommendations for the proper construction of proposed improvements. This report summarizes findings from the geotechnical visual investigations and presents preliminary geotechnical recommendations for the design and construction of this project.

Regional and Site Geology

Arizona State Route 89, known as Oak Creek Canyon Road, lies within the Mogollon Rim, an escarpment that runs east to west through Arizona defining the boundary between the Colorado Plateau to the north and the Basin and Range Province to the south. The Mogollon Rim is approximately 200 miles long and ranges between 2,000 and 3,000 feet in height. Erosion has slowly modified the nature of the Rim, leaving behind scenic canyons and buttes.

The Mogollon Rim is comprised of a variety of geologic formations. The Rim is capped with basalt flows from nearby volcanoes, such as the House Mountain Shield volcano in the village of Oak Creek. Gravels exist beneath the basalt cap, which were formed by the erosion of additional layers that once topped the region. Below the rim gravels, various formations of sandstone and limestone exist, with a range of erosion resistance and hardness properties.

In the vicinity of the project route, on-site materials consist of the basaltic caprock, as the route generally passes over the top of the Mogollon Rim. Volcanic-derived soils may be acidic and high in sulfates, which is potentially corrosive to buried soil and concrete structures. However, this corrosive behavior was not evident in existing corrugated metal pipe and concrete culverts on site.

Regional and Local Seismic Setting

Arizona State Route 89 lies within the transitional area between the Mogollon Rim and Colorado Plateau, and is traversed by many dormant fault zones. These fault zones generally trend to the northwest and dip to the northeast. The Casner Cabin fault zone is located approximately 3 miles west of the southern limits of the project. The Casner Cabin fault zone forms two sharply defined, narrow grabens on Paleozoic bedrock and Pliocene volcanic rocks with a total displacement of at least 130 feet.

The Phone Booth faults lie approximately one mile north of the Casner Cabin fault zone.

The Phone Booth faults are defined as a moderately defined, narrow graben and narrow horst on trend with and northwest of the graben are formed on upper Miocene and Pliocene volcanic rock with total surface displacement of approximately 100 feet.

The Lake Mary fault zone is located approximately 5 miles east of the northern portion of the project route. The Lake Mary fault zone is the longest of several faults located southeast of the Pliocene-Quaternary San Francisco volcanic field, on the erosion surface cut onto Paleozoic rocks near the Colorado Plateau margin. The Lake Mary fault system displaces uppermost volcanic rocks approximately 450 feet.

Geologic Hazards

Most of the project area is underlain by a thin veneer of forest duff with colluvial sands and gravel derived from volcanic and sedimentary bedrock. Potential geologic hazards that could be associated with roadway construction in these geologic units include corrosive soils, slope instability, rockfall, flooding, and earthquakes.

Volcanic-derived soils may be acidic and high in sulfates, which is potentially corrosive to buried steel structures and concrete structures. Visual inspection of corrugated metal pipe culverts and reinforced concrete culverts did not identify the effects of corrosion degradation or sulfate attack. However, native soils should be tested for their corrosive potential and propensity for sulfate attack.

Seismic related hazards exist at the site and were discussed in the previous section.

Materials prone to liquefaction or amplified ground motions are not expected to be encountered at the project site. Due to the relatively steep topography in confined sections along the route, there is a potential for rockfall generated by earthquakes or freeze-thaw cycles, with sources from bedrock outcrops or from boulders in the hillside colluvium.

There are three major drainages within the project limits. These drainages pass through the roadway embankment by means of reinforced concrete box culverts or a concrete arch drainage structure, as noted in Table 1. These drainages are prone to significant flows during the spring runoff and summer thunderstorm seasons. During periods of heavy flow, these drainages may flood and have the potential to erode the toe of roadway embankments. However, there was no observed damage or scour in the channel or adjacent to roadway embankments.

PROCEDURES AND RESULTS

Site Investigation

A visual inspection and limited subsurface investigation program of the proposed improvements within the project limits were conducted by Central Federal Lands Highway Division (CFLHD) Geotechnical personnel on October 1 and 2, 2012. The visual investigation was primarily focused on the following:

• Characterizing general roadway, structures, and culvert conditions,

• Identifying roadway geotechnical repair areas, and

• Identifying geological and geotechnical hazards.

The limited subsurface investigation program was focused on the investigation of potential cut/fill materials, as well as the completion of a topsoil survey. Seven shovel test pits, at various locations within the project limits, were completed as part of the subsurface investigation program.

Findings

Subsurface conditions along the proposed alignment generally consist of a thin veneer of forest duff and topsoil underlain by silty sand, gravel, cobbles, and boulders derived from volcanic and sedimentary bedrock units. Visual observations indicate that bedrock may be very shallow, less than five feet from the ground surface, in several locations along the route. Volcanic cinders were present on roadway shoulders and have been used as shouldering material along the length of the route. The Arizona DOT has a large stockpile of cinders in their maintenance yard within the project limits. Results of the shovel test pits are provided in Table 2.

Table 2. Shovel Test Pit Findings.

Test Pit Location Depth (ft) Soil Description TP-1 366+50, 40’ L 0.0 – 0.5 Brown, fine silty sand with gravel, cobbles and boulders (shovel refusal at 0.5’) TP-2 323+00, 30’ L 0.0 – 0.5 Red-brown sand with gravel and cobbles

Surface conditions within the identified major drainages along the subject route generally consist of sand, gravel, cobble, and boulder alluvium. Observed boulders varied significantly in size, from 1-foot to 6-foot in diameter, especially in the Fry Canyon and Woody Wash drainages. The Landon Spring drainage is in a meadow area and boulders were not evident at the ground surface.

Material type and physical properties of encountered materials may vary with distance away from pit locations. This variability is due to topography and land forming processes.

Precise extrapolation of ground conditions between test pit locations is not recommended.

Visual inspection of the asphalt pavement along the project route indicated the presence of wheelpath fatigue cracking, but no other signs or areas of significant pavement distress.

No specific locations requiring subexcavation were noted. In 2001 the project route was overlain with a 2 inch thickness of asphalt concrete and a 0.5 inch thick friction course overlay.

RECOMMENDATIONS AND SUMMARY

Site Preparation

Clearing and grubbing of the project sites should be performed in accordance with Section 201 of the FP-03. Shallow bedrock conditions and surficial boulders, ranging between 1 and 2 feet in diameter, should be anticipated and may impact clearing and grubbing operations.

Based on conditions encountered during the subsurface investigation, it is not likely that a significant depth of topsoil will be present along the length of the project. There are localized areas within the project limits that contain a significant depth of topsoil, which may be stockpiled and used throughout the project. These areas are generally located within or adjacent to pasture and meadow areas on the northern extent of the project limits.

Silty sand with gravel is the predominant soil type found along the route. These materials exhibit good characteristics for use as embankment material. Occasionally, there are pockets of silt, which may not meet all the requirements for embankment material. Native soils used in embankments should be compacted to 95% of maximum density according to the requirements of Section 204 of the FP-03.

Grading Requirements

Due to the limited amount of local borrow sources or waste areas, and to maximize cut-to-fill balance on this project, it is anticipated that the majority of roadway excavation will be used in the construction of roadway embankments. It is anticipated that native materials will meet the requirements of embankment fill and bedding material, per Section 704 of the FP-03.

Slope Recommendations

In general, the maximum recommended slope ratios for proposed cut slopes vary between 2V:1H to 1V:1H for those cut slopes constructed in moderately weathered and fractured rock units. For those cut slopes constructed in soil, a maximum slope ratio of 1V:1.5H is recommended. The recommended slope ratio is dependent on the nature of the observed material and the existing stability of the slope.
