About this file
This is a Certified Drug-Free Workplace Report Coversheet issued by the State of West Virginia Purchasing Division, required under West Virginia Code 21-1D-7b. Contractors are required to submit this certified report no less than once per year or upon project completion to the appropriate authority based on contract value. The coversheet serves as a transmittal document that must be attached to the required drug-free workplace report and includes fields for contract identification, vendor contact information, and agency information.
The required report content must include documentation of education and training provided regarding West Virginia Code 21-1D-5 requirements, identification of the United States Department of Health and Human Services-certified laboratory performing drug tests, the average number of employees engaged in construction work on the public improvement project, and detailed drug test results categorized by testing type including pre-employment and new hires, reasonable suspicion, post-accident, and random testing, with the number of positive and negative results for each category. Submission requirements are contingent on contract value: contracts exceeding $25,000 must be mailed to the West Virginia Purchasing Division at 2019 Washington Street East, Charleston, WV 25305, while contracts of $25,000 or less should be submitted directly to the public authority issuing the contract.
