About this file
This document is a draft pricing structure for pharmaceutical distribution fees under the VA Pharmacy Benefit (PPV) 2028 contract. The fee structure establishes six product categories, each with two payment terms (FASTPAY and NET30), creating twelve distinct distribution fee classifications.
The six product categories are: (1) Ultra Expensive medications with WAC prices of $30,000 or greater; (2) GLP-1 products that are injectable, refrigerated, and contain glucagon-like peptide receptor agonists or related mechanisms of action; (3) Specialty medications treating chronic, complex, or rare diseases with minimum four of five characteristics including high list prices, specialist initiation/maintenance, special supply chain handling, non-traditional distribution channels, or significant monitoring requirements; (4) Over-the-counter (OTC) medications available without prescription and FDA-regulated for consumer self-use; (5) VA FSS and VA/DLA national contracts including medical surgical items and medical devices with FDA National Drug Codes (NDC) or National Reimbursement Codes (NRC), plus dietary supplements, blood-glucose monitoring units, and in-vitro diagnostics from Federal Supply Schedule Group 65, Part VII; and (6) WBPG ANDA generics priced based on Wholesale Acquisition Cost with FDA abbreviated new drug applications. The document does not include specific fee amounts for these categories.
