About this file

This is a draft pricing document for the PPV (Pharmaceutical Pricing and Volume) 2028 Distribution Fees structure. The document establishes six product categories with corresponding distribution fee structures, each offering both FASTPAY and NET30 payment terms.

The six categories are: (1) Ultra Expensive products with Wholesale Acquisition Cost (WAC) prices of $30,000 or greater; (2) GLP-1s, defined as non-ultra-expensive injectable products requiring refrigeration with mechanisms of action involving glucagon-like peptide receptor agonists, dual agonists, triple agonists, or amylin receptor agonists; (3) Specialty medications treating chronic, complex, or rare diseases with minimum four of five characteristics including list prices exceeding $3,000, specialist initiation/maintenance, special supply chain handling, non-traditional distribution channels, or significant side effects requiring additional monitoring; (4) Over-the-counter medications safe for consumer self-use without prescription and FDA-regulated; (5) VA FSS, all other VA/DLA national contracts, and medical surgical items and devices with FDA National Drug Codes or National Reimbursement Codes, including dietary supplements, blood-glucose monitoring units, and in-vitro diagnostics on Federal Supply Schedule Group 65, Part VII; and (6) WBPG ANDA generics priced based on Wholesale Acquisition Cost with FDA abbreviated new drug applications. The document provides definitional parameters for each category to enable proper product classification and fee application.
