About this file
This is a Conflicts Disclosure Policy form issued by the Rhode Island Department of Transportation (RIDOT) that establishes mandatory disclosure requirements for prospective vendors bidding on state contracts. The policy requires designated organizational representatives—including owners, directors, principals, officers, board members, employees with decision-making authority, and shareholders with controlling interests—to disclose any family or personal relationships, associations, or connections with current RIDOT employees. The disclosure encompasses relationships by blood, adoption, or marriage, including parents, siblings, children, grandparents, grandchildren, in-laws, step-relations, and cousins. Vendors must complete and submit the Conflicts Disclosure Statement as part of the contract bidding process, and they retain an ongoing obligation to submit updated forms if any changes occur in family or personal relations during the contract term.
The disclosure form serves RIDOT's internal purposes to identify and mitigate potential conflicts of interest at contract inception and throughout the contract process. Notably, RIDOT explicitly states that disclosures made on the form will not prejudice prospective vendors from contract selection or award. Vendors uncertain about whether specific relationships or associations require disclosure are directed to consult RIDOT's Legal Office at (401) 222-6510. By signing the form, vendors certify the accuracy and completeness of the information provided and acknowledge their continuing obligation to report any changes. This policy applies to the open enrollment consultant services opportunity MPA-476 for On-Call Cultural Resources services.
