About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form required by the Rhode Island Department of Transportation (RIDOT) for the MPA-476 On-Call Cultural Resources Consultant Services contract opportunity. The certification document requires prospective participants to attest under penalty of perjury that they and their principals are not currently debarred, suspended, proposed for debarment, or excluded from covered Federal transactions. The form mandates that participants confirm they have not been convicted of fraud or criminal offenses related to public transactions, have not violated Federal or State antitrust statutes, and have not been indicted for relevant offenses within the preceding three-year period. Additionally, participants must certify that no public transactions have been terminated for cause or default within the three-year period preceding the proposal submission.
Prospective participants unable to certify to any statement must list exceptions on the form, noting the applicable parties, initiating agency, and action dates. The form emphasizes that false information may result in criminal prosecution or administrative sanctions, and any noted exceptions will be considered in determining contractor responsibility. Contractors with exceptions must contact RIDOT prior to contract award to discuss their circumstances. The certification serves as a prerequisite for participation in the open enrollment consultant services opportunity for on-call cultural resources work, which may include historic property identification, evaluation, Section 106 compliance documentation, and archaeological, architectural, and genealogical studies for RIDOT projects.
