TRANSITION OUT PLAN TEMPLATE

This template describes the standard structure and content of WCMBP project documents. The WCMBP contractor shall use this template to construct project documents, unless waived by the Government. The contractor shall include each section of the template within the document unless it is described as optional within the template instruction.
This document is constructed in sections, consisting of three parts:
· Example text in plain Arial 12pt text which may be replaced by the Contractor.
· Template instructions (italic or <bracketed italic> text) to provide guidance to the contractor on creating the Transition-Out Plan content
· Defaults (bracketed text) where contractors might place an Agency Name or a Company name.
After using them to understand the structure and content of the document, the contractor shall replace all template instructions with the document content (removing the shaded box outline formatting).
The contractor shall change the document properties “Document Title”, “Document Date”, and “Document Version”, to reflect these throughout the document.
The contractor shall reference content from other WCMBP project documents as required to ensure coordination between documents and reduce redundant text.

1 Introduction < This section describes the document, identifying its purpose, scope, and organization. The Transition-Out Plan details the transition of all Government owned equipment, data, software, service, security certificates and maintenance activities to a new Contractor during the last 24 months of the contract. The plan shall state the approach and processes for disposition of the system in an orderly manner and ensure that the system is properly archived or incorporated into other systems.>
1.1 Background
< This subsection contains any background information required to understand the document.> {The Office of Workers’ Compensation Programs (OWCP) is under the Department of Labor (DOL), which mitigates, through the provision of wage replacement and cash benefits, medical treatment, vocational rehabilitation, and other benefits, the financial burden on certain workers, or their dependents or survivors, resulting from work-related injury, disease, or death.
The WCMBP operation processes bills for four OWCP programs:
• The Federal Employees’ Compensation Act (FECA) program
• The Black Lung Benefits Act (BLBA) program
• The Energy Employees Occupational Illness Compensation Program Act (EEOICPA)
• The Longshore Harbor Workers Compensation Act (LHWCA) }
1.2 Purpose

1.3 Scope
< This subsection describes the scope of the document, including any assumptions, dependencies, constraints, or limitations on the document.> 2 Transition Plan Overview <The Contractor shall provide a Transition-Out Plan to the Government within three (3) months after Government notification of the award of a new contract. This plan shall include, but is not limited to:
• A proposed approach for the turnover of operations.
• All tasks for the turnover of operations.
• A turnover of operations schedule reviewed and approved by the Government.
• Documentation of all WCMBP production data, program libraries, and documentation, including documentation update procedures for the turnover of operations.>
3 Transition Approach < This section elaborates on the transition requirements, to determine the nature and extent of the transition coordination services required and provide information and data, approved by the Government in a timely and professional manner to allow a smooth and effective transition of operations. Examples include:
Service Transition:
• Notifications: Describe a plan for notifying users of the service being transitioned, as well as other affected parties, like those responsible for other interfacing systems.
• Data Transition: Describe a plan for archiving, deleting, or transferring to other systems the government-owned data files and related documentation in the system being transitioned.
• Software Transition: Describe a plan for archiving, deleting, or transferring to other systems the government-owned software, library files and related documentation in the service being transitioned.
• Service Documentation Transition: Describe a plan for archiving, deleting, or transferring to other systems the government-owned hardcopy and softcopy systems and user documentation for the service being transitioned.
• Training: Describe a plan to furnish transition training to staff of the new contractors.> 4 Key Stakeholders < This section contains a description of the various types of stakeholders and their assigned roles.>

5 Scope <Scope is defined as the comprehensive set of deliverables, which together meet the transition services requirements listed above, delivery and sign-off of which satisfy the Transition-Out portion of the contract, and constitute the completion of this project.

5.1 Assumptions

5.2 Constraints
<This subsection lists all constraints imposed by the government. Examples include:

Implementation must be completed within < > months of project start date.

Deliverables must comply with requirements specified in the Performance Work Statement and agreed upon during initial planning sessions with DOL or added through the change management process.

Data must be delivered in approved government formats.
