About this file
This is a District Certification Form from the State of Georgia, establishing collusion and ethical compliance requirements for bidders submitting proposals. The form requires bidders to certify that their proposal is submitted independently without prior understanding, agreement, or connection with other bidders submitting responses for the same materials, supplies, or equipment. Bidders must affirm that their submission is fair and free from collusion or fraud, and acknowledge their understanding that collusive bidding violates State and Federal law and can result in criminal penalties including fines and imprisonment, as well as civil damage awards.
The certification further requires bidders to confirm that the provisions outlined in the Official Code of Georgia Annotated (O.C.G.A.) 45-10-20 et seq. have not been and will not be violated in any respect. The form must be signed by an authorized representative of the bidding entity, who certifies their authority to execute the bid on behalf of the bidder. This certification is based on O.C.G.A. 50-5-67 and serves as a standard ethics and compliance verification document for all state contract opportunities in Georgia.
