About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for the Advanced Metering Infrastructure (AMI) System and Installation Services contract opportunity. The form requires prospective bidders to complete, under penalty of perjury, a questionnaire regarding any history of disqualification, removal, or prevention from bidding on or completing federal, state, or local government projects due to violations of law or safety regulations. Bidders must indicate whether they, any officers, or any employees with proprietary interest in the bidder have experienced such disqualification by selecting either "Yes" or "No."

If a bidder answers affirmatively to the disqualification question, the form requires them to provide a detailed explanation of the circumstances and attach supporting documentation. This disclosure mechanism serves as a compliance and risk mitigation tool for the City, ensuring that only qualified vendors without relevant disqualifying incidents are considered for this significant infrastructure project involving water and electric utility metering systems and associated installation services.
