About this file

This document is a Disqualification Disclosure form in accordance with California Public Contract Code (PCC) §10162, requiring prospective bidders to complete a questionnaire under penalty of perjury. The form asks bidders to disclose whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple yes/no checkbox for bidders to indicate their response, with an instruction to explain circumstances and attach supporting documentation if the answer is "yes". The disclosure is a standard legal requirement designed to ensure transparency and accountability in government contracting by revealing any previous instances of contractual or regulatory non-compliance that might impact a bidder's eligibility for a current project.
