About this file

The document is a Disqualification Disclosure form for the City of Pasadena's Parking Facilities Security Program, adhering to California Public Contract Code (PCC) §10162. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that allows bidders to transparently report any past disqualifications by checking either "Yes" or "No" and providing additional explanation if applicable. While the form itself does not contain specific pricing or contract details, it is part of the City of Pasadena's procurement process for selecting a security services provider for eight public parking facilities, with the intent of establishing a management agreement that emphasizes security, crime prevention, customer service, and safety.
