About this file

The document is a Disqualification Disclosure form required by California Public Contract Code (PCC) §10162, which mandates that prospective bidders complete a questionnaire under penalty of perjury. The form requires bidders to disclose whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that requires a simple "Yes" or "No" response, with an option to explain and attach documentation if the answer is "Yes". The purpose is to provide transparency and ensure the integrity of the bidding process by requiring contractors to self-report any previous issues with government contracts. This disclosure helps government entities assess the reliability and compliance history of potential contractors before awarding a contract.
