About this file

The document is a Disqualification Disclosure form required by California Public Contract Code (PCC) §10162, specifically for the City of Pasadena's Parks, Recreation, and Community Services Department's Fitness and Wellness Contract for Instructors. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a binary yes/no response option, with an instruction to explain circumstances and attach supporting documentation if the answer is affirmative. While the document does not contain specific pricing, contract terms, or detailed requirements, it represents a critical compliance document intended to ensure the integrity and accountability of potential contractors bidding on the City of Pasadena's fitness and wellness instruction contract.
