About this file

This is a Certification of Noninvolvement in Prohibited Activities in Russia or Belarus form required by the State of New Jersey for vendors seeking to enter into or renew contracts with state agencies. Pursuant to N.J.S.A. 52:32-60.1 et seq. (L. 2022, c. 3), all vendors must complete this certification to verify their status on the Office of Foreign Assets Control (OFAC) Specially Designated Nationals and Blocked Persons list. The form applies to any entity seeking to provide goods or services, or purchase bonds or other obligations with a state agency. The certification requires vendors to confirm one of three conditions: that the vendor is not identified on the OFAC list related to Russia and/or Belarus activity; that the vendor is unable to certify non-identification because it appears on the OFAC list; or that while identified on the OFAC list, the vendor's Russia and/or Belarus activities are consistent with federal law, regulation, licenses, or exemptions.

Vendors must provide complete business information including their Federal Employer Identification Number (FEIN), legal name, address, phone number, fax number, and email address, along with the signature and title of an authorized representative and the certification date. If a vendor selects option C, indicating OFAC identification but consistent federal compliance, a detailed description of the compliant activities must be attached. The Department of the Treasury is authorized to take appropriate action for any false certifications, including imposing sanctions, seeking compliance, recovering damages, declaring the party in default, or pursuing debarment or suspension. This certification is required for the City of Newark's procurement for Water Meters Parts, Cold (Bid 2026-13), with bids due by 2:00 pm on Thursday, July 23, 2026.
