About this file

This document is a Data Item Description (DID) standard form (DI-NDTI-80809B) that specifies the required format and content for Test/Inspection Reports used in federal contracting.

The DID prescribes a standardized structure for contractors to document test and inspection results, findings, and analyses to enable government agencies to evaluate compliance with system requirements, performance objectives, specifications, and test/inspection plans. The report format requires a cover and title page with specific identification information including report date and number, contractor details, contract number, type of test/inspection, item identification, test period, requiring government activity, and security classification if applicable. The report must include a table of contents with section titles, page numbers, and illustrations; an introduction containing test/inspection objectives, complete item identification (nomenclature, stock number, model/part/serial numbers, item type, engineering changes, specifications, and manufacture date), and correlated inspection requirements; a summary with significant results, observations, conclusions, recommendations, proposed corrective actions, and identified deviations; and reference documents including prior test reports, test plans, certifications, and input parameters with specified issue dates and amendments. The body of the report must contain complete identification of all test equipment used (nomenclature, model, serial number, manufacturer, calibration status, accuracy data); detailed description of the physical test setup including item location, equipment positioning, sensor placement, and electrical/pneumatic/hydraulic requirements; complete procedure descriptions covering item selection verification and testing sequence; recorded data and test results with comparative matrices; test/inspection conclusions distinguishing between objective and subjective assessments; and recommendations addressing item acceptability, additional testing needs, redesign requirements, and documentation changes. The report must include three authentication certifications signed by contractor personnel and authorized representatives: confirmation that testing was performed per applicable plans and procedures with true and accurate results; identification of requirements previously validated through alternative methods; and statement of whether the item passed or failed acceptability requirements. Appendices should contain voluminous supporting data, drawings, photographs, and referenced documentation not previously provided by the government.
