About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) for the ARFQ DEP22-81 Steele Run II Well Plug project. The form serves as an informational guide outlining common bidding errors and requirements that contractors must address when submitting proposals. The document establishes mandatory pre-bid meeting attendance, signature page completion, valid bid bond submission, and acknowledgment of addenda as critical submission requirements. Bidders must submit proposals prior to the specified bid closing date and time, and failure to meet any mandatory RFQ requirement results in immediate disqualification.
The form identifies immediate disqualification criteria including federal or state debarment, suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Additional disqualification risks exist prior to contract award for contractors with outstanding debt to the state or political subdivisions, workers' compensation or unemployment compensation delinquencies, failure to obtain required bonds and insurance, and failure to supply a West Virginia contractor's license number or signed drug-free workplace affidavit within one day of DEP request. Contractors must also provide a subcontractor listing within one business day of bid opening or upon DEP request. The form emphasizes that all potential bidders must thoroughly review the complete bid documents to identify all actual requirements, as the review form does not constitute a comprehensive list of all bidding requirements or grounds for disqualification.
