About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The document serves as an informational guide for potential bidders on construction projects and outlines common bidding problems and disqualification criteria. The form is not a complete list of requirements for any individual project but rather highlights frequent issues that arise during the construction bidding process. All bidders must thoroughly review the request for quotation, all supporting documents, and related instructions in their entirety to identify actual requirements, as failure to do so may result in bid disqualification.
The document identifies two categories of errors affecting bid eligibility. Immediate disqualification errors include failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply a valid bid bond, failure to meet any mandatory RFQ requirement, failure to acknowledge receipt of addenda if required, failure to submit bids by the deadline, federal debarment, West Virginia state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Additional errors that may result in disqualification before contract award include outstanding debt to the state or political subdivisions, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of request, failure to submit a signed drug-free workplace affidavit within one day of request, and failure to provide subcontractor listings within one business day of bid opening or DEP request.
