About this file

Summary

This is a Small Business Participation Plan (SBPP) annual submission form issued by the City of Phoenix Public Transit Department in compliance with the Disadvantaged Business Enterprise - Neutral (DBE-N) program requirements established under 49 CFR Part 26 by the United States Department of Transportation. The form applies to contractors working on Federal Transit Administration (FTA)-assisted projects and requires annual submission on the anniversary date of contract execution. Contractors must demonstrate good faith efforts to promote small business participation through identified actions including contacting the PTD Compliance Specialist, identifying verified small business suppliers, conducting proactive outreach, sending solicitation letters to potential small business vendors, and maintaining documentation of all contacts and responses. The contract term and specific service details are referenced but not detailed within this form, which serves as an administrative compliance document rather than a project scope document.

Contractors are required to track and report all small business participation monthly through the City of Phoenix Certification and Compliance System at phoenix.gob2g.com. The SBPP requires documentation of current small business relationships including subcontractors, joint venture partners, and suppliers of goods and services with amounts paid during the reporting period. Contractors must also identify future opportunities for small business participation by Scope of Work (NAICS code), estimated timeframe, and known contractors or vendors. A designated contract liaison must submit the updated annual SBPP and other required reports. Supporting documentation must accompany all SBPP submissions. This document does not specify funding amounts, pricing terms, or contract values, as it is a procedural compliance form applicable to the referenced Head Start Birth to Five Professional Development Training contract scheduled from December 2, 2026 through April 5, 2028.
