About this file

This is a Darfur Contracting Act Certification form required by the State of California for bidders and proposers on state and local contracts. The form applies to RFQ 03-0812 for USPO Processing and Distribution, which is an informal competitive solicitation for three cold food vending machines. Bidders with business activities or operations outside the United States within the previous three years must complete this certification to establish eligibility to submit a proposal.

The certification requires bidders to select one of three options: certify that they have no international business activities, declare that they are a scrutinized company with written permission from the Department of General Services to proceed, or certify under penalty of perjury that despite having international operations, they are not a scrutinized company as defined under Public Contract Code section 10476. The form must be completed with the company name, Federal ID Number, and signed by an authorized representative, with the execution location identified.
