The worksheet requires contractors to submit comprehensive details about previous CBOC contracts, including the legal entity name, contract number, type of CBOC services performed (establishing, operating, or both), specific tasks completed during establishment and operation, medical services provided, geographic location, contract dates, average monthly patient volume, and contact information for contract verification. Contractors must also specify whether the reference contract is for their own services or those of a proposed subcontractor, and whether they served as a prime or subcontractor. This worksheet is part of the solicitation for VA Veterans Health Administration's CBOC Services in McComb, Pike County, Mississippi, with the goal of evaluating offerors' prior experience in managing primary care clinics for veterans.
