About this file
This document is a detailed procedural guide for the REACH VET (Recovery Engagement And Coordination for Health - Veterans Enhanced Treatment) program, outlining six key steps for healthcare providers serving high-risk veterans. The steps include receiving notification about a high-risk veteran, optionally accessing a clinical dashboard, re-evaluating the veteran's care comprehensively, considering treatment enhancement strategies, conducting outreach using specific talking points, and documenting interactions in the CPRS (Computerized Patient Record System).
The guide provides specific instructions for each step, such as reviewing screening evaluations, diagnoses, and treatment plans; implementing strategies like enhanced communication, safety planning, and increased monitoring of stressful life events; and using a national provider note template for documentation. The document emphasizes a collaborative approach to care, with providers expected to complete all tasks by the fourth Wednesday of the month and conduct outreach within one week of being notified about a high-risk veteran. The goal appears to be proactive, comprehensive care management for veterans who may require additional support.
