Government furnished information, such as building locations, required by the contractor for the performance of this contract will be provided with each request. All information received remains the property of the United States
Government. The Contractor may be in contact with data subject to the Privacy Act (Title 5 of U.S. Code, Section
552.a). Reports and data shall be identified and safeguarded accordingly. Agency procedures shall be followed. The
Contractor shall ensure that contractor employees assigned to this requirement are briefed annually on properly identifying and handling privacy act data/ information.
There is no requirement for Government Furnished Facilities. If vendor provides “on-site” support, access will be granted as required.
4.1 Payment
Invoices shall be submitted monthly in arears.
5.0 Contractor Responsibilities
The Contractor shall provide shipping as required. Government will not pay any travel expenses as part of this
PWS.
5.1 Quality Control.
The contractor shall establish and maintain a complete quality control program for the performance requirements of this contract. The Contracting Officer’s Representative will evaluate the contractor’s performance based on the
PWS.
6.0 Quality Assurance Surveillance Plan (QASP) The government shall monitor Contractor(s) performance in accordance with the Quality Assurance Plan (QASP).
This Quality Assurance Surveillance Plan (QASP) has been developed to provide the standard of surveillance for Telephony Network Maintenance Services.
This plan outlines procedures and guidelines that will be used to monitor the Contractor’s compliance with submitting deliverables and ensure all work is in accordance with the contract requirements. The Contractor is the owner of the quality control process and is responsible for developing, implementing, and modifying procedures that will produce the desired outcomes and result in performance of work within the required standards. The intent of the QASP is to ensure that the Contractor is complying with its quality control systems.
6.1 This QASP is intended to provide the Government’s Contracting Officer’s Representative (COR) an effective and systematic method of surveillance for each service in the PWS. The Government resources shall have responsibilities for the implementation of this QASP as follows:
Contracting Officer – The Contracting Officer ensures performance of all necessary actions for effective contracting, ensures compliance with the terms of the contract, and safeguards the interests of the United States in the contractual relationship. It is the Contracting Officer that assures the Contractor receives impartial, fair, and equitable treatment under the contract. The Contracting Officer is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.
Contracting Officer’s Representative(COR) – The COR is responsible for technical administration of the contract and assures proper Government technical surveillance of the Contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the Contractor deems may affect contract, price, terms, or conditions shall be referred to the
Contracting Officer for action.
6.2 Contracting Officer’s Representative (COR)
The COR for this contract will be provided via email at time of contract award
The COR will act as the Contracting Officer‘s representative for technical matters, providing technical direction and discussion as necessary with respect to the specification or statement of work, and monitoring the progress and quality of contractor performance. The COR is not an Administrative Contracting Officer and does not have the authority to take any action, either directly or indirectly, that would change the pricing, quantity, quality, place of performance, delivery schedule, or any other terms and conditions of the contract (or delivery orders), or to direct the accomplishment of effort which goes beyond the scope of the statement of work in the contract (or delivery order).
When, in the opinion of the contractor; the COR request effort outside the existing scope of the contract (or delivery order), the contractor shall promptly notify the Contracting Officer (or Ordering Officer) in writing. No action shall be taken by the contractor under such direction until the Contractor Officer (or Ordering Officer) has issued a modification to the contract (or delivery order) or has otherwise resolved the issue.
In the absence of the COR named above (due to reasons such as leave, illness, or official travel), all responsibilities and functions assigned to the COR shall be the responsibility of the Alternate COR acting on behalf of the COR.
6.3 Performance Standards. The QASP provides a systematic method to evaluate the service the Contractor is required to furnish. The COR will assess the Contractor’s performance to ensure the Contractor is performing up to the specified standards. The standards for this Performance Work Statement (PWS) are stated in the Level of Effort
Requirements Summary (PRS) included in this QASP. The standards for this procurement are subjective since specific metric-driven standards are not appropriate.
6.4 Management and Oversight. The QASP is based on the premise that the Contractor and not the Government, is responsible for the management and any Quality Control action required to meet the terms of the contract. The
Level Of Effort requirements recognize that the Contractor is not a perfect manager and that unforeseen requirements and uncontrollable problems do occur. Good management and use of an adequate Quality Control
Plan will allow the Contractor to operate within specific performance requirements. QASPs are designed to be objective, fair, and consistent in evaluating Contractor performance against the provided standards.
6.5 Acceptable Quality Levels (AQLs). AQLs have been established in the Level Of Effort Requirements
Summary and provide performance standards that the Contractor will meet in performing the contract. If the contractor meets the required AQL, it will be paid the monthly amount agreed upon in the contract. In the event the
Contractor does not meet the established AQLs, the KO has the right to exercise the negative incentives stated in this QASP.
6.6 Primary Method of Surveillance/Evaluation. The COR will evaluate the contractor’s performance using one or more of the following surveillance methods:
a. 100% Inspection: Inspection where specified characteristics of performance are examined and tested to determine conformance with defined requirements in the Performance Work Statement (PWS).
b. Random Inspection: Sampling method in which each unit of the population has an equal chance of being selected.
c. Customer Surveys and Feedback: A review method where customers and end users are surveyed to provide input on specific service levels.
6.7 Negative Incentives If any of the performance requirements do not meet the AQL set in the PRS table, the
COR shall document the discrepancy(s) and shall notify the Contractor promptly, and the KO, on a monthly basis, for appropriate action. When the performance is below the AQL standard, the Government may implement negative incentives that include: increased surveillance, reduction in contract price, and/or decision not to exercise next option.
6.8 Notification. The COR will notify the KO, in writing, of unacceptable quality levels. The KO will promptly provide written notification of discrepancies on a monthly basis, or immediately if the situation significantly affects the well-being of the project. The Contractor will be given the opportunity to respond, in writing, to each discrepancy.
6.9 Response. The Contractor shall provide a written response to the cause and the corrective action to prevent recurrence within ten (10) working days after receipt of the KO’s notification. The Contractor should cite specific quality assurance program procedures or new procedures instituted to prevent recurrence.
6.10 Ratings. The following criteria will be used for performance evaluations: Exceptional, Satisfactory, Marginal, or Unsatisfactory. Upward or downward arrows may be used to indicate an improving or worsening trend insufficient to change to the assessment status.
a. Exceptional. Indicates performance clearly exceeds contractual requirements. The area of evaluation may contain a few minor problems for which corrective action appears highly effective.
b. Satisfactory. Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.
c. Marginal. Indicates performance meets contractual requirements. The area of evaluation contains one or more serious problems for which corrective actions have not yet been identified, appear only marginally effective, or have not been fully implemented.
d. Unsatisfactory. Indicates the contractor is in danger of not being able to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains one or more serious problems for which the corrective actions appear ineffective.
6.11 Documentation. Documentation used and referenced to perform surveillance will consist of monthly reports, contractor plans and procedures, schedules, customer feedback and contract data requirements.
a. Recording Observations. The COR shall use Surveillance Logs to record information on scheduled observations and deficiencies noted. Each observation is recorded on the log. The documents then become a formal record for reference. The contractor is to be told each time that a deficiency is found during scheduled observations. The COR makes a notation on the Surveillance Log of the date and time the deficiency was discovered, then, the COR asks a contractor site manager to initial the notation, documenting notification of the deficiency only.
b. Potential Unacceptable Performance. The COR must identify the specific service that is unacceptable and the possible causes, and ask a number of questions which if answered, will probably pinpoint the source of the problem.
c. Documenting Unacceptable Performance. The COR must attempt to resolve the problem with the contractor. The original log and the attempted solution (along with an evaluation of results) are forwarded to the Contracting Officer through the COR.
Based on the severity of the discrepancy and the success of the solution, the COR will notify the KO. The contractor shall complete a response to the unacceptable evaluation if requested by the KO or by its own choice.

7.0 BASE ACCESS INSTRUCTIONS
Marine Corps Installations across the region are transitioning to a new government-owned access control system called Defense Biometric Identification System (DBIDS). This new and enhanced system replaces RAPIDGate and
MCESS for contractors and other non-DoD-affiliated personnel requiring installation access. DBIDS is already installed at more than 320 sites with over 5 million registered personnel and unlike previous systems, this one is a free service.
Current RAPIDGate or MCESS credentials can easily be switched out for a DBIDS credential and new contractors can apply for a DBIDS credential using the same vetting requirements as previously required. RAPIDGate/MCESS credentials will be honored until September 30, 2018. After this date, all contractors will be required to have a
DBIDS credential in order to access MCB Camp Lejeune and MCAS New River. In order to reduce wait time when exchanging your RAPIDGate/MCESS credential, it is highly recommended each applicant pre-register at https://dbids-global.dmdc.mil/enroll#!/.
Here's what you need to know:
CURRENT DOD ID CARD HOLDERS
-If you currently hold a valid Common Access Card (CAC), dependent ID card, reserve ID card, or retiree ID card, issued by the Defense Enrollment Eligibility Reporting System (DEERS), no further action is required.
-You will be automatically enrolled in DBIDS when your ID card is initially scanned at the gate. Expect a one-time delay of 3-6 seconds as the DBIDS system initially registers your information.
CURRENT CONTRACTORS
- Beginning June 5, 2018, RAPIDGate/MCESS card holders will be required to present a second form of government-issued identification to the gate sentry, along with the RAPIDGate/MCESS credential, in order to access MCB Camp Lejeune and MCAS New River.
- DBIDS credentials will be issued beginning June 25, 2018
- DBIDS credentials will be valid for the duration of your contract, not to exceed five years; however, re-vetting of individuals' information will occur each year.
- Contractors can exchange their current RAPIDGate/MCESS credential for a DBIDS credential without re-vetting, however they will be required to bring the following items:
Current RAPIDGate/MCESS credential
Second form of government-issued identification (driver license, passport, etc.)
Company Contract/Letter of Authorization
New DBIDS credentials will be valid for the remainder of the contract (not to exceed 5 years), but individuals will be re-vetted each year.
All contractors (MCB Camp Lejeune and MCAS New River) will be able to exchange their current
RAPIDGate/MCESS credential at the Camp Lejeune Contractor Vetting Office, Building TT-13, next to the Wilson Gate. NOTE: If a contractor requires flight line access, they must exchange their credential at
Building AS-187 next to MCAS New River's Main Gate entrance. Only contractors needing flight line access will be serviced at AS-187.
Flight line access will still be granted at AS-302 using the procedures already in place.
NEW CONTRACTORS
DBIDS credentials will be issued beginning June 25, 2018
New contractors requiring access for over 60 days will receive a credential valid for up to one year.
New contractors requiring access for less than 60 days will receive a temporary paper pass.
REMINDER: Beginning June 5, 2018, all personnel in possession of a RAPIDGate/MCESS credential will also be required to provide a second valid U.S. government form of identification. Beginning June 25, 2018, DBIDS credentials will be issued or exchanged in place of a RAPIDGate/MCESS credential. The last day
RAPIDGate/MCESS credentials will be authorized for entry onto the installation is September 30, 2018.
More information on DBIDS can be accessed by current CAC holders at https://dbids.dmdc.mil/#!/resources.
If you have questions regarding the exchange of a current RAPIDGate/MCESS credential for DBIDS or you are a new contractor seeking enrollment into DBIDS, please contact the Contractor Vetting Office at 910-451-
0283/0284/0285.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible quoter whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
The term “offeror” or “offer” as used in FAR 52.212-1 shall be understood to mean “Quoter” and “quote,” respectively. Further, the term “award” shall be understood to describe the Government’s issuance of a purchase order in response to a request for quotes (RFQ).
BASIS OF AWARD is “Best Value” to the Government. The Government will award a single contract on “an all or none basis”resulting from this request for quotes to the responsible vendor whose quote, The term “offeror” or
“offer” as used in FAR 52.212-1 shall be understood to mean “Quoter” and “quote,” respectively. Further, the term “award” shall be understood to describe the Government’s issuance of a purchase order in response to a request for quotes (RFQ).
BASIS OF AWARD is “Best Value” to the Government. The Government will award a single firm fixed-price contract resulting from this request for quote to the responsible vendor whose quote, conforming to the RFQ; will be most advantageous to the Government, price and other factors considered.
Evaluation Factors: (1) Technical Capability including all subfactors, (2) Past Performance; and (3) Price.
The Government will award a single contract resulting from this request for quotes to the responsible vendor whose quote, conforming to the RFQ; will be most advantageous to the Government, price and other factors considered.
Quotes will be evaluated in an efficient and minimally burdensome fashion. The Contracting Officer reserves the right to evaluate quotes without the use of formal evaluation procedures such as establishing a competitive range, conducting discussions, scoring quotations, and to make an award to other than the lowest quote. However, the
Government reserves the right to award no contract at all, depending on the quality of the quotes submitted.
Quotes will be evaluated using a price/technical tradeoff. This process is appropriate because it is in the
Government’s best interest to consider award to other than the lowest price. When combined, all non-cost factors are significantly more important than the price. In evaluating the technical portion of the quote, the technical evaluators will evaluate the following factors and subfactors as described in the “Basis of Award”. The factors of
Technical Capability and Past Performance are weighted equally. All technical subfactors are weighted equally.
Technical Capability and Past Performance, when combined, are significantly more important than the price.
The Government intends to evaluate quotes and award a contract without discussions with vendors. Vendors should not expect requests for clarification or additional information from the Government. Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions with vendors if later determined by the Contracting Officer to be necessary and in the best interest to the Government. The Government may reject any or all quotes if such action is in the public interest; and may waive informalities and minor irregularities in quotes received.
***NOTE Any quoter who receives a rating of Unacceptable on any technical factor or subfactor will be rated unacceptable overall and cannot receive the award.***
EVALUATION. The Government intends to make a single award to satisfy this requirement. Award will be made on the initial evaluation of quotes received in response to this RFQ. Therefore, quoters are cautioned to ensure that initial quotes contain your best terms from a technical and price standpoint. The following factors will be used to evaluate quotes:

Efforts that have not yet been completed shall have at least six (6) months of performance prior to the date of issuance of this solicitation. If a quoter does not request past performance feedback from its customers on the basis that it does not possess a recent/relevant past performance record, then the quoter shall affirmatively state so in its proposal.
Questionnaires regarding the quoter’s own past performance as a prime only will be accepted.
Quoters lacking relevant past performance history will not be evaluated favorably or unfavorably on past performance and will receive a neutral rating.
Quoters shall follow-up to ensure that the Government receives at least one (1), but not more than three (3) completed questionnaires.
It is the quoter’s responsibility to ensure that their customers complete Attachment Four (4) Past Performance
Questionnaire and return it DIRECTLY to Ms. Tracy Fulks, Contract Specialist, tracy.fulks@usmc.mil, 910-451-

The quote shall demonstrate an understanding of and ability to meet all of the requirements set forth in the RFQ’s performance work statement (PWS). The Quoter shall provide sufficient detail to substantiate the validity of all statements. The Quoter shall demonstrate that the quote is in compliance with the requirements, terms, and conditions of the RFQ. Comprehensive responses to the requirements of the RFQ are required to enable the
Government to evaluate the Quoter's understanding of and capability to accomplish the requirements of the PWS.
General statements that the Quoter can or will comply with the requirements, that standard procedures will be used, that well known techniques will be used, or paraphrasing the solicitation’s requirements in whole or in part, will
NOT constitute compliance. Failure to conform to any of the requirements of the solicitation may form the basis for rejection of the quote.
Brochuremanship is not desired. Clarity, completeness, and conciseness are essential and the overall quality of the quote will be evaluated in the context of being representative of the Quoter’s services. A Quoter’s quote is presumed to represent the best efforts to respond to the solicitation.
Since only the quote submitted in response to this solicitation will be evaluated, Quoters should make certain that their quote is complete with regard to the information provided. Data previously submitted, or presumed to be known, e.g., previous projects performed for the Government, will not be considered in the evaluation unless that information is physically contained in the quote.
Quotes shall be presented in three (3) separate volumes:
Volume I (Technical)
Volume II (Past Performance) to be created by the Contract Specialist
Volume III (Price and Solicitation Documents)
VOLUME I SHALL NOT CONTAIN ANY PRICING DATA. Format requirements:

Contractor’s name. Pages shall be numbered consecutively within each volume showing volume and page. For example, page 9 of Volume I would be numbered I-9 and page 10 of Volume II would be numbered II-10.
Quoters are advised that the Government may selectively verify any information provided in the quote. Quoters are cautioned that they are responsible to ensure that material that they desire to have evaluated for a particular section is contained in that specific section.
All quotes shall include the following information in the technical volume.
Volume 1: Technical Capability - See EVALUATION FACTOR 1
The Quoter's technical volume submission shall include all relevant supporting information required or specified in the Solicitation. This Volume shall be limited to 15 double sided pages. Cover sheets, table of contents, and other front matter are not included in the page limit.

Assessment Reporting System (CPARS) reports, Past Performance Questionnaires and/or data independently obtained from other government and commercial sources.
Contract Performance Assessment Reports (CPARs) and Questionnaires (Attachment 4) will be the primary customer feedback data used to evaluate the vendor’s past performance. The Past Performance Information
Reporting System (PPIRS) will be the primary paperless method used to retrieve the CPARS for this source selection. For all contracts identified that do not have a CPARS, The vendor shall forward a copy of Past
Performance Questionnaire (Attachment 1) to that contract’s Procuring Contracting Office (PCO). Please forward all questionnaires within two weeks from this solicitation’s release date to allow adequate time for the recipient to respond. Note that the vendor should not reference this solicitation’s number when distributing the past performance questionnaires to customers. The vendor shall include instructions for the customers to send completed questionnaires within two weeks of its receipt via e-mail to Ms Tracy Fulks, at tracy.fulks@usmc.mil. The vendor shall ensure that all points of contact are current and include an email address and phone number. The vendor shall not conduct follow-up actions with regard to this questionnaire, other than to confirm receipt by the recipient. The
Government may contact customers identified by the vendor to obtain follow-up Past Performance Information as needed.
Vendors lacking relevant past performance history will not be evaluated favorably or unfavorably on past performance and will receive a neutral rating.
Offerors are reminded that the Government reserves the right to use information to evaluate past performance obtained from sources other than those identified by the vendor. The Government may send additional questionnaires and/or obtain information from sources identified by the Government evaluation team.
To evaluate price, quotes will be evaluated using a price/technical tradeoff. This process is appropriate because it is in the Government’s best interest to consider award to other than the lowest price.
