About this file

This is a Past Performance Questionnaire (PPQ) template used by the Marine Corps Installations National Capital Region-Regional Contracting Office (MCINCR-RCO) for source selection evaluation under Request for Quote M00264-26-Q-0053 for Laboratory Testing Services. The questionnaire must be completed by government assessors who served as points-of-contact on the contractor's referenced past performance contract and submitted to teri.bischof@usmc.mil and cynthia.guthridge@usmc.mil no later than June 8, 2026 at 10:00 AM ET with the email subject line "M00264-26-Q-0053 PPQ Source Selection Sensitive." Responses must use 10-point Times New Roman font only.

The PPQ evaluates contractor performance across four primary categories using a five-tier rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory): Quality of Performance (conformance to contract requirements, specifications, and standards of workmanship); Schedule Performance (timeliness, milestone completion, and deliverable submission); Management Performance (responsiveness, subcontract management, program management, personnel management, and integrated product team participation); and Cost Control, applicable only for non-firm-fixed-price contracts (cost estimates, efficiency, invoicing, and forecasting). Assessors complete sections requesting contract scope description, complexity analysis, and overall ratings with supporting rationale, as well as identification of any government contribution to performance problems. The completed questionnaire is marked as source selection sensitive and controlled unclassified information.
