About this file
The document is a Past Performance Questionnaire for a federal contract related to hemodialysis services at the VA Medical Center in Columbia, Missouri. The questionnaire is designed to collect detailed performance feedback about a contractor's previous work, with ratings on a 6-point scale covering quality of product/service, price control, timeliness/schedule, business relationships, and key personnel. The form must be returned by August 21, 2025, to Stephen Showers at stephen.showers@va.gov, and requires comprehensive evaluation of the contractor's performance across multiple dimensions, including service conformity, reporting accuracy, personnel qualifications, management effectiveness, and communication.
The associated federal contract opportunity (Solicitation 36C25525Q0252) involves on-site hemodialysis services at the VA Medical Center, with a dialysis suite containing 3 beds/bays. Historical treatment volumes indicate approximately 86 one-to-one hemodialysis treatments, 221 two-to-one treatments, 41 peritoneal dialysis treatments, and 40 CRRT (Continuous Renal Replacement Therapy) treatments per month. A site visit is scheduled for August 1, 2025, from 10:00 AM to 12:00 PM EST at 800 Hospital Drive, Columbia, MO, with Jessica Furlong as the point of contact.
