The document serves as a certification requirement for contractors bidding on the auditorium lighting replacement project. Contractors must affirm that they are not an "Iran linked business" as defined under Michigan Public Act No. 517 of 2012 and commit to maintaining this status throughout contract performance if awarded.

The affidavit establishes compliance penalties and enforcement mechanisms for false certifications, including civil penalties of up to $250,000 or twice the contract amount, whichever is greater, plus investigation costs and attorney fees. Additionally, any person found to have submitted a false certification becomes ineligible to bid on proposals for three years from the determination date. The document requires execution by an authorized contractor representative and notarization before a notary public, with spaces provided for contractor name, authorized officer signature, title, date of acknowledgment, notary information, and county details.
