About this file
This document is a Past Performance Request for Proposal (RFP) Attachment for Solicitation Number M0026426R0001, specifically designed for the Wounded Warrior Outreach and Resource Support Services contract issued by the United States Marine Corps. The attachment provides a structured template for offerors to document up to three past performance records, requiring detailed information across multiple evaluation categories including general contract information, recency, relevancy, complexity, magnitude, and quality metrics.
The template mandates comprehensive documentation for each past performance record, including contract numbers, record names, awardee details, government points of contact, contract agencies, NAICS codes, performance dates, contract scope, contract type, performance complexity, total contract value, annual experience value, full-time employee utilization, and CPARS quality ratings. Critically, the document emphasizes precise mapping of past performance scope to the current solicitation requirements and warns that incomplete or improperly documented records will not be considered during evaluation. Offerors must also submit a Past Performance Questionnaire (RFP Attachment 3a) if CPARS data is unavailable for a specific record.
