About this file

This is a Medical Center Policy (MCP) from the West Palm Beach VA Healthcare System establishing requirements for communicating test results to providers and patients, effective January 23, 2025, with recertification scheduled for January 31, 2030.

The policy implements VHA Directive 1088 and establishes timeframes and communication protocols for test result notification across multiple service lines. Critical test results requiring immediate action must be communicated within specific timeframes: radiology critical results within 60 minutes, pathology and laboratory critical results within 30 minutes for outpatient non-WHEN hours and to on-call providers during WHEN hours. Test results requiring no action must be communicated within 14 calendar days, while results requiring action must be communicated within 7 calendar days. The policy defines critical results across radiology (ectopic pregnancy, intracranial hemorrhage, aortic dissection, pneumothorax, and other life-threatening conditions), laboratory services (chemistry values such as glucose <50 or >450 mg/dL, sodium <120 or >155 mmol/L, and various drug levels), hematology, microbiology, transfusion medicine, anatomic pathology, point-of-care testing, and cardiology (positive stress tests, malignant arrhythmias, pericardial effusion). Communication methods include direct telephone contact with read-back confirmation, TEAMS messaging, encrypted messaging, or face-to-face contact. The policy assigns specific responsibilities to ordering providers, diagnostic practitioners, nursing staff, pharmacy, imaging, and service chiefs for ensuring compliance and monitoring. For inpatient, outpatient, and emergency department settings, escalation procedures are defined when primary contacts are unavailable, routing notifications to on-call providers, coverage providers, section chiefs, or the emergency department as appropriate.
