About this file

This document is a Culvert Inspection Report prepared for the Massachusetts Bay Transportation Authority (MBTA) for a specific culvert structure located in Chelsea, Massachusetts, with the structure identification number C-09-021 (A5C). The inspection was conducted on August 18, 2024, as part of Task Order No. 2 under Contract No. Z94PS30, with the report prepared by a Prime Consultant based in Norwood, MA. The inspection revealed no critical findings, though the report includes recommendations for structural repairs, monitoring, and maintenance to be addressed within the next two years as part of the MBTA's Bridge Management Program.

The report includes multiple appendices covering routine culvert inspection details, potential critical findings notification, underwater inspection reports, preliminary repair cost estimates, and structure inventory and appraisal sheets. The preliminary repair cost estimate is noted as being based on 2024 costs and would require adjustments for future dates. While specific repair recommendations are partially redacted, the document emphasizes the importance of programmatic maintenance and monitoring of the culvert structure to ensure its ongoing operational integrity and safety.
