About this file
This document is a Contract Extension Amendment issued by the Arizona Department of Environmental Quality (ADEQ) for Contract No. CTR067133, involving Energy & Environmental Advisors Inc. as the contractor. The amendment is specifically related to UST (Underground Storage Tank) Inspections and is being processed through ADEQ's Procurement department located in Phoenix, Arizona.
The contract extension extends the existing agreement to July 31, 2026, with the contractor required to maintain current insurance certificates meeting specified requirements. ADEQ is authorizing the continuation of services into the new state fiscal year, with work to be invoiced according to the contract terms. Invoices for work performed through July 31, 2024, must be submitted by August 30, 2025, and will be paid within 30 days of receipt. The amendment indicates that all other contract requirements, specifications, terms, and conditions remain unchanged, and a new purchase order will be issued once the budget is loaded for the fund account.
