REQUEST FOR PROPOSAL (RFP) # BPM005267
UST Inspections
Past Performance Questionnaire
Contractor: Williams & Co Consulting Inc
Project:
Please email the completed Past Performance Questionnaire to weiner.jeremiah@azdeq.gov
Reference Check provided by (Name/Entity): Samuel Broussard, Louisiana Department of Environmental
Quality, Underground Storage Tank Division
On a scale of 0 to 6 (0 being the lowest & 6 being the highest) how would you rate this Contractor’s performance on the following:
1. How would you describe and rate quality of work performed by this Contractor on your project?
This contractor had a very good grasp of UST compliance knowledge when we first met up in late 2009 to discuss the material that I wanted in our operator training presentation and work books. They have been very cooperative in adapting their training whenever we have changes in our regulations, policies, and enforcement. They have been extremely respectful not to make any changes to program without my written permission. They have been very professional when I periodically audit their work and they are extremely receptive to any changes that I recommend.
Please provide rating from 0 to 6: 6
2. Was the project performed by this Contractor completed on time?
The initial training that we contracted to them was performed on time and all periodic changes that we have required have been made to the presentation and work books on time.
3. Was the project completed within budget?
They are a subcontractor to my main contractor so I have no visibility on this issue, although my main contractor has utilized them exclusively since we started this project in 2009.
4. Please describe the quality of deliverables submitted:
The presentations that this contractor gives and the work books that they provide are very well thought out professionally put together. The presenters that they use in my state, Randy Jackson and Fernando Calle, are exceptional presenters. I have reviewed every evaluation of the 10,810 UST mailto:weiner.jeremiah@azdeq.gov
Page | 2 Rev.12-18-2018 owners/operators that they have trained and have not had a bad review of them yet. In fact, UST owners/operators often note how good and professional these two presenters are.
5. Did the Contractor provide adequate training and documentation so that what they implemented was sustainable?
Yes. The presentations and work books are excellent, and they change the format every three years to keep it fresh for the attendees (UST owners and operators have to be retrained every 3 years in Louisiana).
6. Was Contractor’s staff responsive to solving of any problems that may have occurred on your project?
Yes.
We have made quite a few changes to the presentations, work books, online registration process, and recordkeeping process over the past 13 years and the contractor’s staff have been very responsive to making changes.
7. Would you be willing to contract with this Contractor again? Yes
To the Arizona Department of Environmental Quality: The undersigned has certified all information contained in the Past Performance Questionnaire is current and factual.
