About this file
This is a Costs and Payments form for a City of Phoenix procurement for LED display installation at the Phoenix Convention Center's Lower Level. The project scope includes furnishing and installing two 8.5 ft x 15 ft LED video wall displays and three 5 ft x 8 ft LED advertisement displays with 1.9mm pixel pitch specifications compatible with the existing 22Miles signage system. A pre-offer conference and mandatory site visit are scheduled for September 25, 2025, with written inquiries due by October 1, 2025, and offer submissions due on October 15, 2025, at 10:00 am. The contract term will commence approximately January 1, 2026, and extend for five years. The scope of work encompasses display installation, system integration, testing, operator training, and a comprehensive five-year warranty and maintenance plan covering parts, labor, and repairs.
The payment terms default to 0% net 45 days, calculated after receipt of invoice or final acceptance of products, whichever is later. Contractors may alternatively offer a prompt payment discount of a specified percentage for 30-day payment or maintain the 0% option for 45-day payment; discount offerings will not be evaluated in the price assessment. Contractors may also elect enrollment in the City's Single Use Account (SUA) Program for immediate payment upon invoice approval, provided they accept and absorb all merchant bank transaction costs without passing charges to the City. The City reserves the right to purchase up to 100 percent of specified quantities. The procurement emphasizes Disadvantaged Business Enterprise (DBE) and small business participation, requiring contractors to submit a Small Business Participation Plan within 30 calendar days after contract award and maintain compliance with federal regulations under 49 CFR Part 26, including documented DBE outreach efforts and monthly expenditure reporting in the City's Certification and Compliance System.

COSTS AND PAYMENTS
(please complete and return with the submittal)
PAYMENT TERMS & OPTIONS: Vendors must choose an option, if a box is not checked, the
City will default to 0% - net 45 days:
____ Contractor offers a prompt payment discount of either ______% - 30 days or 0% – 45 days - to apply after receipt of invoice or final acceptance of the products (invoice approval), whichever date is later, starts the 30 days. If no prompt payment discount is offered, the default is 0%, net 45 days; effective after receipt of invoice or final acceptance of the products, whichever is later. Payment terms offering a discount will not be considered in the price evaluation of your offer.
____ Contractor may be paid immediately upon invoice approval, if enrollment is made to the
Single Use Account (SUA) Program, administered by the City’s servicing bank (“Bank”). By checking this box, the vendor accepts transaction costs charged by their merchant bank and agrees not to transfer to the City those extra charges. The City will not pay an increase in our services for the SUA charges; if an audit uncovers an upcharge for the SUA charges the vendor will owe the City all costs. The vendor may opt-out of the SUA program once, but then may not rejoin during the same contract term. For more information about the SUA program or to enroll, send email to mailbox.sua@phoenix.gov.
