About this file

This is a Contractor Security Acknowledgement Form issued by the State of South Dakota's Bureau of Information Technology (BIT) for a Medical Cannabis Patient Registry, Establishment Licensing system contract. The form requires contractors and their employees to acknowledge compliance with the State of South Dakota Information Technology Security Policy prior to accessing non-public state technology infrastructure or information. The contractor must certify understanding of nine key security requirements, including protection of state technology and information assets from unauthorized access, disclosure, modification, deletion, and usage; adherence to state and federal data confidentiality and handling regulations; and prompt reporting of policy violations to the designated state contact and BIT Help Desk at 605-773-4357. The form serves as a binding agreement whereby the contractor's authorized representative affirms authority to commit all company employees to the security policy terms.

The security acknowledgement establishes that access to state technology infrastructure constitutes a privilege subject to revocation at BIT management's discretion, with access automatically terminating upon contract conclusion unless otherwise specified in writing. Policy violations may result in privilege revocation and potential breach determinations by the State, with individuals found non-compliant subject to disciplinary action. The contractor assumes full responsibility for ensuring all current and future employees comply with the attached Information Technology Security Policy and must return the signed form to their designated BIT Contact prior to commencing work on the Medical Cannabis Patient Registry system development project.
