About this file
The document is a Contractor Responsibility Information form from the City of Tucson, Arizona, designed for an Apparent Low Bidder to complete and submit by 4:00 p.m. local time on the fifth full business day after bid opening. The form requires the contractor to provide comprehensive information about their company's history, performance, legal standing, and capacity to complete the project. The City of Tucson seeks to evaluate the contractor's responsibility by examining factors such as financial resources, performance record, technical expertise, and ability to meet contract requirements.
The form requires detailed disclosure of the contractor's past performance, including any liquidated damages, contract terminations, legal actions, bond claims, and potential legal or bankruptcy proceedings. Additionally, contractors must provide their current Experience Modification (EMod) Rating and describe three representative projects similar to the current project's scope, including project details, owner references, work descriptions, subcontractors, project values, and construction schedules. The form emphasizes that failure to provide sufficient information may result in the contractor being found non-responsible, and any erroneous statements could lead to contract termination.
