About this file
This is a compliance affidavit form required by Florida Statutes Section 287.138 for entities bidding on contracts with Hillsborough County Public Schools (HCPS). The affidavit must be completed and executed by an authorized representative of any proposer or contractor submitting a bid, proposal, or quote for HCPS procurement opportunities, including the Roof Drain Services solicitation (25494-RFP-MST). The form serves as a mandatory prerequisite for bid acceptance, with proposals rejected if the completed affidavit is not submitted. The solicitation targets experienced vendors to provide comprehensive roof drain maintenance and repair services across HCPS facilities, with contracts to be awarded to three primary vendors serving West, Central, and East geographical areas. Vendors must submit detailed pricing using a provided Excel spreadsheet reflecting rates for standard and non-standard hours for both Journeyman and Apprentice technicians, with an estimated annual usage of 2,500 standard hours and 200 non-standard hours. The question deadline is October 9, 2025, and the final proposal due date is October 23, 2025, with the initial contract term beginning upon Board approval and potential automatic renewals for up to four additional one-year periods.
The affidavit prohibits contracting with entities that are owned by, have a controlling interest from, or are principally located in designated foreign countries of concern, specifically the People's Republic of China, the Russian Federation, the Islamic Republic of Iran, the Democratic People's Republic of Korea, the Republic of Cuba, the Venezuelan regime of Nicolas Maduro, or the Syrian Arab Republic. A controlling interest is defined as possessing 25 percent or more of voting interests or profits. The form requires attestation under penalties of perjury that the legal company name meets none of the prohibited criteria under Florida Statutes. The procurement encourages small business participation through the Small Business Encouragement Program (SBEP) with evaluation points awarded based on the percentage of work allocated to HCPS-registered small business enterprises. Vendors must provide a one-year warranty on all goods and services, with pricing structured as firm, fixed price per unit that includes delivery, trip, fuel surcharges, and service call charges. Additionally, vendors are required to submit a Vendor Registration Form and a Vendor Affidavit Regarding the Use of Coercion for Labor and Services as part of the proposal submission requirements.
