About this file
This is a compliance affidavit required by Florida Statutes Section 287.138 for entities seeking to contract with Hillsborough County Public Schools (HCPS) for tutoring services. The affidavit must be completed and executed by an authorized representative of the proposer prior to bid submission, as contracts will not be accepted without this completed form. The tutoring services contract opportunity seeks to establish a vendor bank to support student academic needs aligned with Florida State benchmarks in core subject areas, with the initiative targeting improved academic performance for Title I sites and students in Exceptional Student Education (ESE) and English Language Learners (ELL) programs. HCPS intends to award contracts to the highest-ranked proposer and multiple alternate awardees.
The affidavit requires proposers to certify that their legal company does not meet any of three disqualifying criteria under Florida law: ownership by a foreign country of concern, controlling interest held by a foreign country of concern, or organization under the laws of or principal place of business in a foreign country of concern. For purposes of this affidavit, "foreign countries of concern" are defined as the People's Republic of China, the Russian Federation, the Islamic Republic of Iran, the Democratic People's Republic of Korea, the Republic of Cuba, the Venezuelan regime of Nicolas Maduro, and the Syrian Arab Republic. A controlling interest is presumed when an entity or person directly or indirectly holds voting rights or profit entitlements of 25 percent or more. The declaration is made under penalties of perjury pursuant to Florida Statutes Section 95.525.
