About this file
The documents are construction-related bid submission forms from the West Virginia Department of Environmental Protection (DEP), specifically a Construction Bid Submission Review Form dated February 12, 2024. The forms outline critical requirements and potential disqualification criteria for contractors bidding on state construction projects. Key disqualification reasons include failure to attend mandatory pre-bid meetings, not supplying a valid bid bond, missing mandatory RFQ requirements, missing bid submission deadlines, federal or state debarment, outstanding environmental enforcement actions, and unresolved vendor complaints.
The documents include detailed templates for bid bonds and a drug-free workplace conformance affidavit, which contractors must complete to be eligible for bidding. Bid bonds require a minimum of 5% of the total bid amount and must be executed by a surety licensed in West Virginia. The drug-free workplace affidavit mandates that companies maintain a written drug-free workplace policy in compliance with West Virginia Code §21-1D. Additional potential bid disqualification factors include outstanding debt to the state, workers' compensation or unemployment compensation delinquencies, failure to obtain required bonds or insurance, and not providing a West Virginia contractor's license number or subcontractor listing within the specified timeframes.
