Consolidated State Law Affidavit Summary
This is a Consolidated State Law Affidavit form required by the City of Gainesville, Florida for vendors bidding on the Grounds Maintenance at Evergreen Cemetery contract. The affidavit requires bidders to attest compliance with multiple Florida statutes covering public entity crimes, scrutinized companies, human trafficking prohibitions, E-Verify employment verification, foreign country of concern restrictions, and economic incentive prohibitions. Bidders must certify under penalty of perjury that neither they nor their affiliates appear on the Convicted Vendor List, Discriminatory Vendor List, or Antitrust Vendor List; are not engaged in boycotts of Israel or listed on scrutiny lists related to Sudan, Iran, Cuba, or Syria; do not use coercion of labor; maintain E-Verify registration and compliance; and are not owned by, controlled by, or organized under the laws of foreign countries of concern including China, Russia, Iran, North Korea, Cuba, Venezuela, or Syria. The form must be notarized and signed by an authorized representative of the bidding entity, with execution required prior to contract award for the October 1, 2026 through September 30, 2028 grounds maintenance contract (with possible extension through September 30, 2031).
The affidavit serves as a compliance prerequisite for all contractors seeking to work with the City of Gainesville and establishes the legal framework for contract eligibility verification. Bidders must complete the form truthfully and acknowledge that any falsification may result in contract termination. The attestations address both federal and state-level regulatory requirements, including employment authorization verification through E-Verify and restrictions on contracting with entities having business connections to designated foreign countries or engaging in prohibited international business practices. No exceptions or waivers are provided within the affidavit document itself, and all bidders responding to the May 7, 2026 bid deadline must submit a fully executed and notarized affidavit as part of their proposal package.
