About this file
This document outlines Compliance Verification Documentation requirements for a state contract opportunity, specifically for the State of Ohio. The document details mandatory submission requirements for bidders, including a comprehensive list of documents that must be provided alongside a quote, such as an Acord Certificate of Liability Insurance, Bidder's Certification Form, and various state-specific certifications. The documentation emphasizes strict prohibitions on offshore services and investments, particularly highlighting restrictions on purchasing services located outside the United States and a specific prohibition on transactions involving Russian institutions or companies.
The compliance documentation mandates several key verifications, including current address confirmation, affirmative action program verification, Secretary of State Charter/Registration Number, current Bureau of Workers' Compensation (BWC) Certification, and potential minority or specialty trade certifications (MBE, EDGE, VBE, WBE). Bidders must also complete a Contractor/Subcontractor Affirmation and Disclosure Form affirming their understanding of service location restrictions. The document underscores the state's right to recover funds for any services performed outside approved locations and requires continuous disclosure of any changes in service locations during the contract performance period.
