About this file
This compliance forms document establishes the disclosure and certification requirements for bidders and contractors on a State of Florida contract opportunity for wet glazing services at a ten-story OUC office building located at 100 West Anderson Street in Orlando, Florida. The project scope involves performing wet glazing of the exterior curtain wall and window systems with the intent to improve air and water infiltration performance through installation of a continuous, properly bonded sealant system at all glazing joints. The forms do not specify response dates, due dates, site visits, bidder meetings, award dates, or contract term details.
The compliance forms require bidders to disclose any reprimands, suspensions, defaults, terminations, or litigation within the past five years through a dispute disclosure section. Additionally, bidders must submit a sworn statement under Florida Statutes 287.133 and 287.135 certifying that neither the bidder nor any of its officers, directors, employees, or affiliates have been convicted of public entity crimes, and confirming the bidder is not listed on the Scrutinized Companies with Activities in Sudan List or the Scrutinized Companies with Activities in the Iran Petroleum Section for contracts valued at $1 million or more. Bidders must also complete an exceptions form documenting any clarifications, deviations, or variances from the specifications and requirements, with the understanding that failure to disclose exceptions may result in proposal rejection and that no exceptions will be accepted after submission.
