About this file
This document is an Attachment A containing supplemental terms and conditions for the Illinois Department of Human Services (IDHS) Invitation for Bid (IFB) for sidewalk removal and replacement services at the Mabley Developmental Center located at 1120 Washington Avenue, Dixon, Illinois. The project involves the removal and replacement of existing sidewalks at the facility. While specific quantities, response deadlines, site visit schedules, and award dates are not detailed in this attachment, the document establishes the contractual framework and compliance requirements that vendors must meet to be considered responsive bidders for this public works project.
The supplemental terms require vendors to comply with multiple Illinois statutory requirements including employment of at least 90% Illinois laborers during periods of excessive unemployment, prevailing wage rate compliance as published by the Illinois Department of Labor, and submission of a surety bond in an amount not less than the vendor's bid amount from an A.M. Best rated surety company. Additionally, vendors must maintain an Illinois primary office for construction workers, carry hazardous occupation builder's risk insurance, provide a United States Department of Labor Apprenticeship and Training Certificate or Program Letter, and submit either a written Equal Employment Opportunities policy or an acceptable workforce projection detailing employee demographics and trade categories.
