Summary of Task Assignment DC018V under DEP Contract HW552

This document is a Task Assignment Notification Form issued by the Florida Department of Environmental Protection (FDEP) under DEP Contract No. HW552 (Bureau of Waste Cleanup Services, dated December 10, 2010). The task assignment authorizes Professional Service Industries, Inc. (PSI), an Intertek company based in Tampa, Florida, to conduct annual groundwater monitoring activities at the Hampton Cleaners facility located at 5317 Gunn Highway, Tampa, Hillsborough County, Florida (FDEP Site No. ERIC_4713). Key personnel include DEP Contract Manager Killian Talley and PSI Contractor Representative Nana Westmark.

The scope of work consists of two subtasks: Project Management and Annual Groundwater Sampling and Analysis and Report. PSI will collect groundwater samples and elevation measurements from nine monitoring wells (six shallow wells, three deep wells, and two recovery wells), conduct laboratory analysis for volatile organic compounds using EPA Method 8260, and prepare a comprehensive Groundwater Monitoring Report with GIS/ADaPT package deliverable. The total task assignment value is $6,950.38 on a fixed-price basis, with no retainage, comprising 50 labor hours. The final deliverable is due June 18, 2021, with task assignment execution through October 1, 2021. Invoicing occurs upon FDEP receipt and approval of the deliverables. Subcontractor costs include $115.00 for investigative derived waste (IDW) management and disposal through Petrotech Southeast, Inc., and $858.00 for laboratory analytical services.

HW552 DC018V

Task Assignment authorizes PSI to conduct annual groundwater monitoring at the Hampton Cleaners site.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Sampling and Analysis and Report Subtask includes costs and labor for PSI to conduct groundwater sampling along with analysis and reporting. Field event will be comprised of the following activities; collection of depth to water will be measured from site monitoring wells, field parameters for geochemical data, eleven (11) groundwater samples for analysis of volatile organic compounds via EPA Method 8260B, and management of investigative derived waste. Upon receipt of the groundwater analytical data, PSI will generate a Groundwater Monitoring Report outlining and describing the field event, procedures, and analytical results. The report will also provide conclusion and recommendations. IDW will be transported and disposed by PetroTech Southeast as the lowest responsive and responsible bidder.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $6,950.38 with 50 labor hours.

PSI may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Re: Proposal for Annual Groundwater Monitoring

Hampton Cleaners 5317 Gunn Highway Tampa, Hillsborough County, Florida

FDEP Facility No. 29 9500922 / ERIC_4713 PSI Project No. 0552656

Dear Mr. Talley:

Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide Annual Groundwater Monitoring services for the Hampton Cleaners at 5317 Gunn Highway, Tampa, Florida. Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion.

PROJECT BACKGROUND

The Hampton Cleaners is located at 5317 Gunn Highway, Tampa, Florida. The drycleaner business is longer in operation and currently is a drop off and pick up facility for cleaning. The drycleaner space is located in the Hampton Plaza shopping complex in a residential / commercial developed area of Citrus Park, near Tampa. The site is bordered to the north by other tenants of the shopping complex, to the east by residential land use and to the south and west by a parking lot that has been recently developed as an Auto Zone retail facility. A private charter school is located in the same complex footprint but in a separate building.

A RAP Addendum was implanted by PSI in December 2013 and January 2014, consisting of SVE well installation, groundwater amendment application and sewer line replacement. The property owner was concurrently responsible for removing some of the impacted soil around the stormwater grate behind the building.

mailto:Killian.Talley@FloridaDEP.gov

FDEP – Hampton Cleaners

An SVE / air injection system was transferred from the Department’s Lakeland storage facility to the Hampton site and installed in May through June 2014. The remediation system operated from June 2014 to December 2017. Confirmation soil samples collected in July 2018 revealed no detections of the contaminants of concern above Chapter 62-777, FAC Soil Cleanup Target Levels (SCTLs), and the system was shutdown.

The most recent groundwater monitoring in April 2020 revealed the groundwater plume appears to be migrating southwest. PSI recommend continuing with the annual groundwater monitoring as a Post Active Remediation Monitoring strategy.

PSI has developed the proposal contained herein to continue the groundwater monitoring program for 2021. The recommended network is based on previous results and in accordance with our most recent groundwater report.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of conducting groundwater sampling and analysis, along with a report deliverable. The results will be used to evaluate current groundwater quality conditions.

Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

2. Annual Groundwater Sampling and Analysis and Report

a. Mobilization to the site.

b. Collect groundwater samples and groundwater elevation measurements from the following wells:

i. Shallow wells: MW002, MW006, MW007, MW010, MW012, and MW014.

ii. Deep wells: DMW008, DMW009, and DMW015,

iii. Recovery wells: RW003 and RW005.

c. Collected groundwater samples will be laboratory analyzed for Volatile Organic Halocarbons (VOH) by EPA Method 8260. The analysis will be performed using a National Environmental Laboratory Accreditation Conference (NELAC) approved laboratory.

d. The investigative derived waste (IDW) will be containerized in a 55-gallon drum for later removal to an approved facility for treatment / disposal.

e. An Annual Groundwater Monitoring Report will be developed which will include current and historical data, with a comparison to state standards and consist of current groundwater elevation and quality date, development of ADaPT package, and recommendations. Conclusions and recommendations will be made, which will include any modifications to the proposed annual groundwater sampling event. The report will incorporate specific recommendations and provide opinions based on professional experience to achieve the most efficient endpoints and closure.
