HW552 DC004T

Task Assignment HW550 DC004T authorizes PSI to oversee and complete an injection event at the Tempo Cleaners site.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Pre-Injection Subtask authorizes labor and related costs for pre-inject activities including coordination with property owner, permitting, set up for field activities, and update health and safety as necessary.

Subtask 3: Baseline Monitoring Sub task authorizes labor and related costs for baseline sampling of the site. Depth to water for site wells will occur along with sampling for VOHs using EPA Method 8260B by a fixed-base laboratory. Certain samples will also be taken for total organic carbon, total dissolved solids, iron, and sodium in accordance with the performance monitoring plan. PSI will discuss results of sampling with the Department prior to further injection activities.

Subtask 4: Injection Subtask authorizes labor and related costs for injection activities. Solution will be mixed with 40,500 gallons of potable water before injection into the 8 injection wells. Along with injection of solution into site, 2-liters of KB-1® Plus culture and 1 liter of KB-1® Primer will be injected into the wells for bioaugmentation purposes. Support for injection will be provided by Cascade who has been selected as injection subcontractor following discussions with the FDEP based upon concerns with injection pressures and subcontractor’s experience with injecting the amendment.

Subtask 5: Performance Monitoring This subtask includes labor and related costs for performance monitoring. Groundwater samples will be analyzed by a fixed-based lab for underground injection control parameters and VOHs as summarized in the Remedial Action Plan Modification. Supplemental monitoring parameters include testing for Dehalococcoides population and volatile fatty acids to help quantify remedial action efficacy.

Tempo Cleaners Page Two

Subtask 6: Evaluation, Reporting Subtask includes labor for PSI to develop an Injection Completion Report including preparation and review of report, figures, as-builts, updated tables, and sign and seal of report. Report will include describing the field event, procedures, and analytical results. The report will also provide conclusion and recommendations.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment Change Order. The final cost for this Task Assignment Change Order is $117,066.41 with 433 labor hours.

PSI may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Re: Proposal – Groundwater Remedial Action Plan Implementation – Revision 1

Tempo Dry Cleaners 233 Oxford Road Fern Park, Seminole County, Florida FDEP Facility ID No. ERIC_5476

Dear Mr. Talley:

Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to implement the Remedial Action Plan Modification – Revision 4 to address impacted groundwater at the Tempo Dry Cleaners in Fern Park, Seminole County, Florida. Included with this proposal is the project history to date, proposed scope of work, cost estimate and a proposed schedule for completion.

PROJECT BACKGROUND

Drycleaning operations at the Tempo Dry Cleaners began in January 1978 and utilized perchloroethene (PCE). In November 1994, a 150-gallon aboveground storage tank (AST) containing PCE was reportedly tipped over. On or prior to November 30, an area approximately 4 feet by 4 feet was excavated, in an attempt to remove the contaminated soil associated with the AST discharge. The excavated soil was later used as backfill. A soil sample collected from the former excavation area had a PCE concentration of 49,000,000 milligrams per kilogram (mg/kg).

In May 1995, the PCE drycleaning operations ceased at Tempo, and the business was sold and converted to a dry drop-off as documented by a FDEP inspection in March 1997. The drycleaner was accepted into the Drycleaning Solvent Cleanup Program in June 1998, and assessment activities conducted by WRS commenced in October 2002.

Tempo Dry Cleaners, RAP Implementation

October 26, 2020, revised June 24, 2021

In February 2007, an air sparge curtain system, consisting of six (6) air sparge wells was implemented at the site. An Interim Source Removal was conducted at Tempo Cleaners in April 2008.

PSI was tasked with providing remediation system start-up in January 2011. Monthly O&M activities commenced in February 2011 and were discontinued in August 2011. A Membrane Interface Probe study was conducted in November 2011 which revealed more specific data to interpret the contamination plume which is widespread and is migrating vertically. The air sparge system appears to have been successful in decreasing contaminant levels in the shallow zone. However, historic efforts to remediate the intermediate zone have had little impact.

Annual groundwater monitoring data from 2013 to 2018 indicate the continued presence of contaminant concentrations above applicable regulatory Cleanup Target Levels. A groundwater PCE concentration as high as 32,000 μg/L has been reported for samples collected from monitoring well EMW012, which was located in the same location as the former PCE AST where the reported discharge occurred (ECS, 2000).

Over the last 15 years or so, groundwater PCE concentrations have generally decreased in the AST spill area and increased in some of the downgradient wells. Contaminant reduction in the AST spill area appears to be attributable to a combination of source removal activities (ISR and SVE), contaminant migration, and natural attenuation.

In April 2015, PSI representatives met with the owner due to planned construction to remodel the former drycleaner space and expand the convenience store. PSI modified the interior SVE trenching system to underground and performed Soil Oxidant Demand (SOD) testing activities on soil samples to evaluate future remedial strategies. Approximately 12 tons of contaminated soil was removed and transported for off-site treatment and disposal. Two vapor points were abandoned, and one shallow well was installed inside the building.

During discussions with the Department in March 2016, various injection point locations were evaluated for PeroxyChem’s EHC® product. EHC® is composed of zero valent iron (ZVI), controlled release carbon and nutrients, which stimulate the chemical reduction and abiotic degradation of the contaminants. A buffer will also be introduced to ensure the pH is maintained above 6 SU. The selected target zone is the interval 40 to 55 feet bls.

A RAPMOD, dated November 2018, revised September 26, 2019 has been submitted to the FDEP. The RAPMOD proposes in situ chemical reduction, which combines abiotic chemical reduction (zero valent iron) and anaerobic bioremediation. The target treatment areas include the original release area and the on-site portion of the groundwater plume that exceeds NADC levels.

Groundwater testing performed in August 2019 revealed a concentration of PCE in temporary point of compliance (TPOC) well BMW034 exceeding the GCTL. The FDEP has therefore requested a new TPOC well to be located downgradient of BMW034. Additionally, FDEP has requested the installation of intermediate depth wells along the western edge of the groundwater VOH plume for horizontal delineation. Four new monitoring wells and eight permanent injection wells were installed in August 2020. Three of the monitoring wells, designated as BMW044 through BMW046, were located on-site, adjacent to and south of the dispenser area. The fourth monitoring well, designated as BMW047, was located off-site to the south in a residential area which was established as the new Temporary Point of Compliance (TPOC). The revised RAP Modification Revision 4 was submitted to the Department October 15, 2020.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of the performance of the Groundwater Injection Events (injection of EHC Liquid, KB-1 Plus bioaugmentation cultures, and associated amendments into eight locations, as outlined in the RAP) and report deliverable, along with baseline and performance groundwater monitoring events post injection and a post injection report deliverable.

Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

2. Pre-Injection Activities

Various tasks will be conducted prior to the actual groundwater injection which will consist of:

• Arrange access with site owner to the site injection areas and common areas for temporary equipment and product storage

• Coordinate with local agencies regarding any permitting requirements

• Obtain permit for use of potable water from a nearby fire hydrant

• Arrange for forklift to be utilized on site for moving the injection reagents

• Receive the injection reagents, unload the injection reagents from the delivery truck, and store the injection reagents on-site beneath a waterproof covering until they are ready to be used.

• Prepare or update a site-specific Health and Safety Plan applicable to the proposed field activities in this RAP

October 26, 2020, revised June 24, 2021 www.intertek.com/building

3. Baseline Monitoring

PSI will perform the baseline monitoring in general accordance with the approved RAP to include the following:

4. Groundwater Injection

Eight permanent injection wells have been installed at the site. The injection well locations and well construction details are indicated on Figure 7 of RAP (RAPMod, Revision 4; PSI, 2020).

Cascade was selected as the injection subcontractor during previous discussions with the FDEP based on the FDEP’s concerns with injection pressures and Cascade’s experience with injecting EHC-L.

PeroxyChem will supply the EHC-L and potassium bicarbonate. The EHC-L will be delivered as two components that will be mixed together in the field. The first component is 100% Emulsified Lecithin Substrate (ELS), which will be provided in 55-gallon drums, with 50 gallons of ELS per drum. The second component is the EHC Liquid Mix, which contains ferrous iron powder, and is delivered as a dry powder in 24.6-pound bags.

SiREM will supply the KB-1 Plus bioaugmentation cultures and KB-1 Primer for preparing anaerobic water.

The sodium ascorbate will be purchased from a retail vendor. Quotes from Cascade, PeroxyChem and SiREM are enclosed with this proposal.

The injection mixture will be either transferred to a feed tank connected to a pump, or directly pumped from the mixing tank into an injection manifold. The injection manifold allows simultaneous injection into up to eight injection points, though field applicability and production rates will be determined by site conditions and what the formation will accept at each injection point. The expected injection flow rate is no more than 5 gallons per minute (gpm).

Injection Procedures

1. Cascade will mix and prepare the EHC-L solution for application.

2. Cascade will supply all necessary injection equipment, including pumps, mixing tanks, delivery manifold, injection heads with flow & pressure gauges, safety bypass valve, PPE and safety equipment for Cascade personnel, and first aid station.

3. Using a diaphragm pump, the Cascade solution will be transferred from the poly tank into the injection wells.

4. Cascade will perform real time reagent distribution monitoring for optimization of the injection design during the injection activities.

5. The visual observance of the volume in the graduated tanks and an in-line flow meter will be used to calculate injection volumes at each injection point.

6. Cascade will provide daily injection logs detailing material quantities, elapsed time, injection pressures, and field observations.

Injection points and surrounding areas will be monitored for any signs of surfacing, and spill prevention/control supplies will be on hand to respond as needed. Injection pressures and flow rates will be monitored at each injection point, and appropriate field adjustments will be made as needed to properly distribute the remedial agents. Potential field adjustments include changes in injection point locations and injecting in stages (i.e., injecting what the formation will accept, then moving on to other points and returning to the original point later).

Dilution water will be obtained from a permitted fire hydrant and pretreated with sodium ascorbate at a mixing ratio of approximately 2.5 pounds per 1,000 gallons (approximately 300 mg/L). DO levels will be monitored and the mixing ratio will be adjusted as needed to maintain DO levels at or below 0.5 mg/L.

A 30-fold dilution of ECH-L is proposed, followed by an adequate volume of chase water to achieve an injection radius of at least 6 feet. The recommended mixing ratio is summarized below and can be scaled as appropriate for the size of the mixing tank:

EHC-L and KHCO3 Mixing Ratio

Unit Qty ELS 100% Liquid Emulsion, 50 gal (460 lb) Drum 1 EHC Liquid Mix, 24.6 lb Bags 4.5 Potassium Bicarbonate (KHCO3) Buffer Pounds 450 Pretreated Water from Fire Hydrant Gallons 1,600

SiREM will supply the KB-1 Plus bioaugmentation cultures and KB-1 Primer. The KB-1 Plus cultures will be directly injected from the shipping vessel into the injection well using an in-line tee fitting. The cultures will be injected between layers of anaerobic water (prepared with KB-1 Primer) to disperse electron donors and protect the anaerobic bioaugmentation cultures. The injection of cultures and anaerobic water will be performed between layers of EHC-L to promote contact between the bacteria and electron donor.

Unit Qty KB-1 Primer Package 1 Water from Fire Hydrant Gallons 250

KB-1 Plus Injection Ratio

Unit Qty KB-1 Plus per Injection Well Liters 2 Anaerobic Water per Well (approximate) Gallons 100

The estimated injection volumes per well and total injection quantities are summarized below, based on a 30-fold EHC-L dilution and a target injection radius of at least 6 feet:

Approximate Injection Volumes Per Well

Unit Qty EHC-L Mixture (Pre KB-1 Plus) Gallons 206 KB-1 Plus and Anaerobic Water Gallons 100 EHC-L Mixture (Post KB-1 Plus) Gallons 206 Chase Water Gallons 4,564 Total Injection Volume per Well Gallons 5,076 Injection Volume per Vertical Foot Gallons 338

Approximate Total Injection Quantities

Unit Qty Number of Injection Wells Each 8 ELS Concentrate Drums 2 EHC Liquid Mix Bags 9 Potassium Bicarbonate Pounds 900 Sodium Ascorbate Pounds 100 KB-1 Plus Liters 16 KB-1 Primer Pouches 4 Anaerobic Water Gallons 800 Dilution Water Gallons 3,200 Chase Water Gallons 36,500 Total Injection Volume Gallons 40,500

Flowcharts illustrating the mixing ratios, injection volumes per well, and total injection quantities are provided in Appendix E of the RAP (RAPMod, Revision 4; PSI, 2020), and are also incorporated in the injection services quote (Cascade) enclosed with this proposal.

The injection of EHC-L and KB-1 Plus may cause a temporary exceedance of the drinking water standard for iron, sodium and total dissolved solids (TDS), although a significant deviation is not anticipated.

Paragraph 62-520.310(8)(c), F.A.C., allows these parameters to temporarily exceed standards in a temporary zone of discharge (ZOD) upon FDEP approval of this RAPMod, provided the RAPMod addresses the duration and size of the ZOD, and groundwater monitoring requirements.

PSI will monitor for the following underground injection control (UIC) parameters prior to injection, and on a quarterly basis thereafter:

• Iron, 0.3 milligrams per liter (mg/L), maximum

• Sodium, 160 mg/L, maximum

• Total dissolved solids, 500 mg/L, maximum

A field deliverable consisting of field notes, logs, a brief summary of injection procedures, and laboratory data will be submitted prior to the final report.

5. Performance Monitoring

Post-injection groundwater quality, UIC monitoring, and injection performance will include one or more of the following analytical parameters during each sampling event: VOHs, iron, sodium, TDS and total organic carbon (TOC). The proposed groundwater monitoring schedule is summarized in Table 7 of the RAPMod, Revision 4; PSI, 2020, with modifications due to contract end dates. The monitoring will only include two quarters, not the original four quarters.

Supplemental performance monitoring parameters included in the KB-1 Plus quote (SiREM) include testing for Dehalococcoides (Dhc) population and volatile fatty acids (VFAs). These tests help quantify and characterize the bioaugmented microbial community and provide indicators of whether electron donor fermentation is occurring.

6. Reporting

An Injection Completion Report will be submitted within 90 days of completing the injection event and monitoring event. The report will include injection drawings certified by a professional engineer registered in the State of Florida.

COST ESTIMATE AND SCHEDULE

The estimated cost to implement the RAP is presented on the attached Cost Estimate and Level of Effort.

All PSI services will be conducted in accordance with the terms and conditions contained within DEP Contract No. HW552, Bureau of Waste Cleanup Services, dated December 10, 2010. Upon authorization, we anticipate performing baseline monitoring in August 2021, injection in September 2021, and quarterly monitoring in December 2021 and March 2022. The final reporting will be in May 2022.

PSI appreciates the opportunity to be of service to the FDEP. If you have any questions, please feel free to contact us at (813) 886-1075.

Well Injections of EHC-L with KB-1 Bioaugmentation Based on the information provided by Intertek-PSI and the remediation design provided by PeroxyChem, Cascade Technical Services (Cascade) will mobilize to 223 Oxford Road, Fern Park Florida. Equipped with custom built injection equipment, reagent mixing equipment, experienced 40-hour HAZWOPER trained crew, material handling equipment, secondary containment, site safety materials and all necessary consumables. There is a total of eight (8) injection wells.

Cascade will provide multiple tanks for mixing reagents and producing deoxygenated (DO) water for injecting and chasing KB-1 cultures. Water will be procured from a nearby fire hydrant, and the Intertek site representative will test the DO parameters with a YSI (or similar) meter prior to injecting. Pressure rates, flow rates and volume totals will be closely monitored and recorded in Cascade injection logs and made available to the Intertek site representative at the end of each day. Injection points and surrounding areas will be monitored for any signs of surfacing, and spill prevention equipment (absorbent materials and wet vac) will be on hand to respond to any such conditions should they occur. Excess “mounding” or surfacing conditions could cause delays to the projected production rate. Cascade will communicate with Intertek-PSI immediately if surfacing conditions could extend the estimated days onsite.

Cascade assumes water for this injection event can be procured from a nearby fire hydrant (within 750 feet of injection area). Should water need to be obtained from another source, this could impact the estimated production rate.

• (8) injection wells

• Total Injection Volume: 40,500 gallons

• Per well volume: 5,076 gallons

• 3 person crew

• Manifold to 8 wells simultaneously

• Assume 5 gpm per well and 40 gpm total flow rate output

• Assume 11,000 gallons injected per day

Cascade will be equipped with a custom-built injection platform, low DO water tank (sodium ascorbate and water mixture), reagent mixing tank (EHC-L mixture), 250-gallon anaerobic water tank (KB-1 Primer mixture), multi-port manifold with individual pressure gauges and flow totalizers, secondary containment and safety devices (shower and eye wash), spill response kits, site control equipment, and a forklift.

• Each injection well will be fitted with a custom injection well head equipped with a pressure relief valve for safe operating procedures to inject the EHC-L /KB-1 amendments.

• A multi-point injection manifold will be used to inject into up to eight (8) wells simultaneously.

• Prior to mixing EHC-L, hydrant water will be treated with sodium ascorbate to lower DO/remove chlorine at a rate of 2.5 pounds every 1,000 gallons of hydrant water.

• EHC-L will be mixed and kept suspended in a mixing tank separate from a DO water holding tank.

• Once the appropriate volume of solution has been injected, bioaugmentation cultures will be injected through a dedicated port designed to keep the culture from coming into contact with the atmosphere. Anaerobic flush water will then be injected to push the culture into the formation.
