Document Summary
This is a Task Assignment Notification Form under DEP Contract No. HW552 issued by the Florida Department of Environmental Protection (FDEP), Bureau of Waste Cleanup Services, to Professional Service Industries, Inc. (PSI), an Intertek company. The task assignment authorizes PSI to conduct annual groundwater monitoring activities at the Rene Cleaners facility located at 2710 Blanding Boulevard, Middleburg, Clay County, Florida (FDEP Site ERIC_4266). The contract was executed in March 2021, with signatures from DEP Task Manager Killian Talley and contractor representative Nana G. Westmark. The assignment is designated as Task Assignment DC015H under Module D000 with no associated grant funding.
The scope of work consists of two subtasks: (1) project management with labor hours automatically calculated based on direct labor costs, and (2) annual groundwater sampling and analysis including installation of a temporary monitoring well, collection of groundwater elevations from ten monitoring wells, sampling of five monitoring wells for volatile organic compounds via EPA Method 8260, management of investigative derived waste (IDW), and development of a comprehensive Groundwater Monitoring Report with GIS tables and an ADaPT package deliverable. The total task assignment value is $6,165.85 on a fixed-price basis with no retainage, comprised of $470.17 for project management and $5,695.68 for groundwater sampling activities. Direct labor costs total $4,231.47 (54 hours), with additional expenses for travel ($466.00), laboratory analysis ($462.00), field equipment ($340.14), other direct costs ($78.09), and IDW disposal services ($150.00) provided by Clark Environmental. The final deliverable—a Groundwater Monitoring Report and GIS/ADaPT package—was due August 27, 2021, with completion of the task assignment through December 3, 2021.

HW552 DC015H
Task Assignment authorizes PSI to conduct annual groundwater monitoring at the Rene Cleaners site.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Annual Groundwater Sampling and Analysis and Report Subtask includes costs and labor for PSI to conduct groundwater sampling along with analysis and reporting. Field event will be comprised of the following activities; collection of depth to water will be measured from site monitoring wells, field parameters for geochemical data, five (5) groundwater samples for analysis of volatile organic compounds via EPA Method 8260B, and management of investigative derived waste. Upon receipt of the groundwater analytical data, PSI will generate a Groundwater Monitoring Report outlining and describing the field event, procedures, and analytical results. The report will also provide conclusion and recommendations. IDW will be transported and disposed by Clark Environmental as the lowest responsive and responsible bidder.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $6,165.85 with 54 labor hours.
PSI may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Re: Proposal for Annual Groundwater Monitoring Rene Cleaners 2710 Blanding Boulevard Middleburg, Clay County, Florida FDEP Facility ID No. 10 9502097 / ERIC_4266
Dear Mr. Talley:
Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide temporary monitoring well installation and annual groundwater monitoring for the Rene Cleaners at 2710 Blanding Boulevard, Middleburg, Florida. Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion.
PROJECT BACKGROUND
The Rene Drycleaner, located in a strip mall, began drycleaning operations in January 1988. The site was accepted into the Drycleaning Solvent Cleanup Program based on a soil sample with PCE at 2100 micrograms per kilogram (µg/kg). The site is currently a drop-off facility but the drycleaning equipment remains.
According to the site assessment, the site lithology consists of interspersed lenses of sand and clay, with a layer of stiff clay at 24 to 50 feet bls. The assessment also revealed that no contaminants of concern were identified below 24 feet below land surface (bls).
A Remedial Action Plan for short term Dual Phase Extraction (DPE) was implemented in May through June 2001. The DPE was evaluated as ineffective due to short-circuiting of the system believed to be the result of a severely cracked surface seal. In March 2002, a source removal near the sewer line was conducted. During the annual groundwater monitoring, a Modified Active Gas Sampling (MAGS) event was performed which identified residual PCE beneath the facility.

PSI Proposal 0552-336491 FDEP – Rene Cleaners
Although a short-term soil treatment strategy was recommended, additional remediation has not been performed at the site.
Groundwater monitoring has occurred in 2011, 2012, 2013, 2014 and 2017 with results indicating no VOH compounds present at concentrations exceeding GCTL criteria.
In October 2014, soil sampling was also performed to evaluate if the site soil had achieved Soil Cleanup Target Levels (SCTLs) and could be closed. Four soil samples were collected and analyzed near previous locations where elevated concentrations of VOH compounds were detected. One soil sample (SB015, 1 ft.) beneath the former dryclean machine exhibited elevated PCE concentrations with an SPLP sample also above Leachability criteria.
In 2018, PSI installed a temporary monitoring well (MW013) inside Rene Cleaners and adjacent to the former dryclean machine. MW013 was subsequently sampled along with monitoring wells MW003R, MW005 and DMW009R as part of the annual groundwater monitoring program.
Analytical results from MW003R, MW005 and DMW009R indicated no VOHs above GCTL criteria.
However, PCE, TCE and vinyl chloride were present in MW013 at 5.6 ug/L, 12 ug/L and 1.6 ug/L, respectively, above the GCTL criteria.
In 2020, another temporary monitoring well was installed near the former dryclean machine which revealed vinyl chloride at 1.7 ug/L in MW013R. Based on these results, the site will not qualify for a clean closure. The Department may wish to negotiate a conditional closure with the property owner, but until that time, we recommend continuing with groundwater monitoring at the Rene Cleaners.
PROPOSED SCOPE OF WORK
The proposal contained herein consists of conducting groundwater sampling and analysis, along with a report deliverable. The results will be used to confirm current groundwater quality conditions. Specifically, the proposed scope of work consists of the following subtasks:
1. Project Management
This scope is provided for in the Level of Efforts sheets.
2. Annual Groundwater Sampling and Analysis
a. Mobilization to the site.
b. Install a temporary well at the former MW013 location.
c. Collect groundwater elevation measurements of ten monitoring wells (MW001, MW002, MW003R, MW005, MW008, DMW009R, DMW010, MW011, MW012 and MW013).
PSI Proposal 0552-336491 FDEP – Rene Cleaners
d. Collect groundwater samples from five monitoring wells: shallow wells MW001, MW003R and MW005, the proposed temporary well, and deep well DMW009R.
The groundwater samples will be analyzed by EPA Method 8260 for Volatile Organic Halocarbons (VOHs) by a laboratory in accordance with protocols approved by the National Environmental Laboratory Accreditation Laboratory Conference (NELAC).
e. The decontamination fluids, soil cuttings and other associated investigative derived waste will be stored in 55-gallon drums, labeled and transported off-site to an approved facility for disposal and treatment.
f. Development of GIS tables and deliverable,
g. Development of an ADaPT package deliverable.
