About this file
This is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division, Department of Administration, applicable to the Nevada State Board of Dental Examiners' request for proposals to provide lobbying services. The form serves as a prerequisite certification document that vendors must complete and submit as part of the contract award process. Bidders are required to certify their lobbying activities and compliance with federal regulations governing the use of appropriated funds for influencing government officials in connection with federal contracts, grants, loans, and cooperative agreements.
The certification requires vendors to declare under penalty of perjury that no federal appropriated funds have been or will be paid to influence any agency officers, members of Congress, or their employees in connection with the contract award. If non-federal funds have been used for lobbying purposes, vendors must complete and submit Standard Form-LLL disclosing such lobbying activities. Vendors must also ensure that all subcontractors and sub-recipients at every tier include identical certification language in their award documents and certify accordingly. Failure to file the required certification subjects violators to civil penalties ranging from $10,000 to $100,000 per violation. The form requires completion of fields for the solicitation number, vendor name, authorized signer name, signature, and date prior to submission.
