About this file
This document is a Certification and Contract Offer form from Washington County, Oregon, dated May 4, 2020, related to a procurement opportunity for property tax statement inserts. The form serves as a standardized proposal submission document that requires vendors to certify various legal and ethical compliance statements before submitting a bid for the county's Department of Assessment and Taxation.
The document requires bidders to affirm several key certifications, including non-discrimination commitments, tax law compliance, and disclosure of any potential debarment or legal issues. Vendors must confirm they have not engaged in collusion, have met Oregon tax obligations, and have no disqualifying legal history. The form includes provisions for extending pricing to other public agencies, defining resident bidder status, and provides space for vendor contact information and signatures. Washington County reserves the right to terminate the contract if the selected vendor fails to comply with tax laws or misrepresents their qualifications.
