About this file

This document is a Certification Regarding Lobbying form for contracts, grants, and cooperative agreements, applicable to federal transactions. The certification requires the undersigned to affirm that no federal appropriated funds have been or will be paid to influence or attempt to influence any congressional or agency officials in connection with the awarding of federal contracts, grants, loans, or cooperative agreements. The form mandates that if any non-federal funds are used for lobbying purposes, the applicant must complete and submit Standard Form-LLL, Disclosure Form to Report Lobbying.

The certification is a material representation of fact, with submission being a prerequisite for entering into the transaction as imposed by Section 1352, title 31, U.S. Code. Failure to file the required certification can result in civil penalties ranging from $10,000 to $100,000 for each failure. The document requires signatures from two authorized officials - an Executive Director and a Chief Financial Officer - and is to be completed for a specific grantee and grant program (noted as NSLP or SFSP in the template).
