About this file

This is a Certificate of Partial Payment form used in conjunction with the Clay County Sheriff's Office Building 500 Renovation project in Fleming Island, Florida. The form serves as the vendor's certification mechanism for progress payments during the interior renovation contract, which encompasses selective demolition, architectural improvements, HVAC modifications, electrical installation, plumbing modifications, and audiovisual equipment installation to create two new training rooms, with optional alternates for a conference room and cubicle/office space. The mandatory pre-bid meeting is scheduled for March 13, 2026, at 11:00 AM at the project site (1845 Town Center Boulevard, Fleming Island, Florida), with bids due March 30, 2026, at 4:00 PM and bid opening on March 31, 2026, at 9:00 AM through the Clay County e-Procurement Portal. Substantial completion is required within 345 days from the Notice to Proceed, with liquidated damages assessed at $500.00 per day for delays past the final completion date.

The Certificate of Partial Payment requires the vendor to certify that all work has been performed and materials applied in accordance with contract terms, and establishes strict payment pass-through requirements: the prime contractor must remit payment to subcontractors and suppliers within 10 days of receipt, and subcontractors must remit payment to their hired subcontractors and suppliers within 7 days thereafter. All contractors must meet comprehensive insurance requirements including $1,000,000 Commercial General Liability per occurrence, $1,000,000 business automobile liability, and workers' compensation at statutory limits. A five percent bid bond of the base bid amount is required, with Performance and Payment Bonds mandated for projects exceeding $200,000 under Florida Statutes Section 255.05. Award will be made to the lowest responsive bid from a responsive and responsible bidder, with the county reserving the right to award the base bid with or without alternates based on available funding, and all work subject to a two-year warranty on workmanship and materials from the date of substantial acceptance.
