16. REMARKS
Planning event Minutes and Site Survey results, provide documentation of technical information and data required to record joint contractor/government decisions and agreements reached during conferences, formal reviews, inspections, or audits. The format shall be formatted neatly and
IAW AR 25-50.
Content. Minutes of each planning event shall include the following as a minimum:
a. A title page containing:
(1) type of conference meeting and date.
(2) identification of system/equipment, training courses, exercise unit, exercise number, contract number as appropriate.
(3) space for signatures of the designated representatives of the contractor and the Government.
b. The purpose and objective of the planning event.
c. The conference/meeting location.
d. A summary of the discussions, decisions, agreements reached, and directions of the conference/meeting or individual subcommittee thereof.
e. A list of attendees by name, rank, rate/grade, or position/activity represented, and phone numbers and e-mail.
f. Copies of action item sheets for each action to be implemented as a result of the planning event. Format of the action sheets shall be as specified by the planning event lead.
g. An update copy of the planning event agenda that reflects the actual agenda for the planning event conducted.
h. The required elements of information specified by the conference/meeting chairman which are applicable to the type of conference/meeting conducted and other data and information as mutually agreed between the contractor and the other data and information as mutually agreed between the contractor and the Government.
Administrative notes as required.
Copies of slides and/or handouts used during the planning event shall be included in the Conference/Meeting Minutes.
Classification. Conference/Meeting Minutes shall be classified in accordance with the classification of the material covered within the conference/meeting. Where limited classified items are included, the Conference/Meeting Minutes shall be constructed so that classified portions are in a separate document referenced in the Conference/Meeting Minutes.

Planning Minutes shall be approved by the chief of operations group for each operations group.
Planning event Minutes and Site Survey results:
(1) Coordination. Commitments made by exercise units and MCTP.
(2) Facilities. Discuss specific facilities being offered for use by MCTP, suitability for use, etc. Provide sketches and floor plans as necessary. Verify Power requirements.
(3) Equipment. Analysis of equipment and other support provided to MCTP. Does it meet requirements to simulation systems and mission command systems to support training and staff for over 500 to 1000 personnel?
Any shortcomings or issues?
(4) Administrative notes. Other administrative information such as phone numbers for sending clearance information, instructions for entering a secured area, parking passes, shipping information and anything needed to support a WFX.
(5) Conclusions and Recommendations.
(1) Unresolved issues
(2) List date and location for next major meeting or milestone.
(3) Any major problems or concerns
Planning Minutes shall be approved by the chief of operations, (CHOPS) for each operations group. This may change if Command directed.
