SCOPE OF ENGAGEMENT

In 300 words or less, describe the scope of the engagement in terms of:

The business purpose, problem, and/or objectives that the project was designed to address The functionality that was desired by the customer The user community that was served by the solution.

APPROACH

In 500 words or less, describe the Technical Approach that was employed to implement the solution. Discuss at a high level the technologies that were implemented and the reasons for selecting these technologies. If SAS was used, please list the SAS applications that were installed in the solution and the approximate number of licenses purchased for each application.

CHALLENGES ADDRESSED

In 500 words or less, describe the key challenges that the project team faced during this project and explain how they were successfully addressed.

BENEFITS REALIZED

In 300 words or less, describe the key benefits that the customer realized as a result of this project.

SERVICE LEVELS ACHIEVED

Provide a copy of the Service Level Agreement for this customer and provide externally verifiable documentation that all service levels have been achieved during the past 12-month period. If any service levels have not been achieved, please provide the following explanation, in 100 words or less, for each unachieved service level:

Describe why the service level was not achieved Identify the period of time during which the service level was not achieved Indicate what steps were taken to rectify the problem Confirm that those steps succeeded in resolving the problem. (If remediation is ongoing, please indicate the date by which the Offeror believes the service level will be achieved.)
